化痰通络解痉汤与泻阴补阳针法联合治疗脑卒中后瘫痪效果观察

Effect of Huatan Tongluo Jiejing Decoction Combined with Purging Yin and Tonifying Yang Acupuncture Therapy on Paralysis after Stroke

目的：探讨化痰通络解痉汤与泻阴补阳针法联合治疗脑卒中后瘫痪的效果。方法：选择2014年2月—2016年12月在我医院神经内科就诊的脑卒中后痉挛性瘫痪的患者100例,按照随机数字的方法分为对照组和观察组,每组50例,对照组患者采用西药治疗与日常康复训练的方法进行干预,观察组患者在对照组患者干预方法的基础上,再加用化痰通络解痉汤和泻阴补阳针刺法进行干预,对治疗效果进行对比。结果：两组患者在治疗前的整体功能状态和日常生活能力的评分数据差异均不具有统计学意义（P〉0.05）,在经过治疗之后,两种评分均出现了显著的上升,数据差异具有统计学意义（P〈0.05）,其中观察组治疗后的数据显著高于对照组,数据差异具有统计学意义（P〈0.05）,两组的治疗效果数据差异具有统计学意义（z=4.135,P〈0.05）,观察组的治疗效果显著优于对照组,对照组与观察组在治疗前、治疗后1个月、治疗后2个月的血清中的谷氨酸（Glu）和γ氨基丁酸（GABA）的含量数据比较,采用重复测量数据的方差分析,结果：（1）不同时间点间的数据差异具有统计学意义（F=13.346、14.251,P〈0.05）,（2）对照组与观察组的数据差异具有统计学意义（F=15.312、22.104,P〈0.05）,数值上谷氨酸项目上观察组显著低于对照组,γ氨基丁酸项目上观察组显著高于对照组,（3）对照组与观察组的数据与时间之间存在交互效应（F=12.694、13.557,P〈0.05）。结论：利用化痰通络解痉汤和泻阴补阳针法联合西医的方法对脑卒中后的痉挛性瘫痪患者进行治疗,可以提升患者的治疗效果,改善患者的氨基酸类神经递质情况,值得在临床范围内进行推广。

Objective：To investigate the effect of Huatan Tongluo Jiejing Decoction combined with purging Yin and tonifying Yang acupuncture therapy on paralysis after stroke. Methods：100 cases in February 2014 to December 2016 in my hospital stroke neurological treatment of spastic paralysis after patients,according to randomly divided into control group and observation group,each group 50 cases,the control group gave Western medicine treatment and routine rehabilitation training in patients with intervention,patients in the observation group were treated by combination of resolving phlegm and dredging collaterals,relieving spasm soup,purging Yin and tonifying Yang acupuncture,to compare the therapeutic effect. Results：Two groups of patients in the whole function of rating data before treatment and daily life ability were not statistically significant（P〈0.05）. After treatment,the two scores were significantly increased,with statistically significant difference（P〈0.05）,the data of observation group after treatment was significantly higher than control group. Data had statistically significant difference（P〈0.05）,with statistically significant treatment effect of data between the two groups（Z=4.135,P〈0.05）,the treatment effect of the observation group was significantly better than the control group,the control group and the observation group before treatment,1 month after treatment,compared serum glutamic acid of 2 months after treatment（Glu）and gamma aminobutyric acid（GABA）content,used variance analysis of repeated measurements：（1）Data had statistically significant differences at different time points（F=13.346,14.251,P〈0.05）.（2）Difference data in control group and the observation group had statistically significant（F=15.312,22.104,P〈0.05）,observation group was significantly lower than the control group,Glu in GABA in observation group was significantly higher than the control group.（3）There were interaction effects between the control group and the observation group on the data and time（F=12.694,13.557,P〈0.05）. Conclusion：On spastic paralysis after stroke patients were treated by combination of Huatan Tongluo JiejingDecoction and relieving spasm soup,purging Yin and tonifying Yang acupuncture combined with Western medicine,can improve the therapeutic effect of patients,improve the amino acid neurotransmitters of patients,it is worthy of promotion in the clinical range.
