180例脑梗死静脉溶栓患者中医证候演变及疗效分析

Analysis on the Traditional Chinese Medicine Syndromes Evolution and Curative Effect of Cerebral Infarction Patients undergoing Intravenous Thrombolysis Therapy for 180 Cases

目的探讨脑梗死静脉溶栓患者中医证候演变情况及其与临床疗效关系。方法回顾性分析180例接受静脉溶栓的脑梗死患者,分析其中医证候演变情况,并比较不同中医证候患者溶栓治疗疗效差异。结果溶栓治疗后风证、火证、痰证、瘀证患者明显减少,气虚证和阴虚证患者明显增加,差异具有显著性(P<0.05);风证、火证、痰证、瘀证患者溶栓总有效率优于气虚证、阴虚证患者,经有序资料Ridit分析提示,风证、痰证、瘀证患者疗效较好;实证患者溶栓治疗疗效优于虚证患者(P<0.05)。结论溶栓治疗后脑梗死患者中医证候改变明显,且不同中医证候患者溶栓治疗疗效不同。

Objective To explore the evolution of TCM syndromes in patients with cerebral infarction undergoing intravenous thrombolysis and its relationship with clinical efficacy. Methods Retrospective analysis of 180 patients with cerebral infarction undergoing intravenous thrombolysis was made. The evolution of TCM syndromes was analyzed,and the curative effect of thrombolytic therapy in different TCM syndromes was compared. Results Patients with wind-syndrome,fire-syndrome,phlegm-syndrome and stasis-syndrome decreased obviously after thrombolytic therapy,patients with qi deficiency-syndrome and yin deficiency-syndrome was significantly increased,and the difference was significant（ P〈0. 05）. The curative effect of patients with wind-syndrome,fire-syndrome,phlegm-syndrome and stasis-syndrome was higher than that of patients with qi deficiency-syndrome and yin deficiency-syndrome. After the ordinal data Ridit analysis,patients of windsyndrome,fire-syndrome,phlegm-syndrome and stasis-syndrome was of good curative effect; empirical efficacy of thrombolytic therapy in patients with superior syndrome（ P〈0. 05）. The effect of thrombolytic therapy of strong syndrome patients is better than that of deficiency syndrome（ P〈0. 05）. Conclusion The TCM syndrome of thrombolytic treatment with cerebral infarction patients changes obviously,and different TCM syndromes patients are of different effect after thrombolytic treatment.
