川芎嗪联合低分子肝素钠对早发型重度子痫患者妊娠结局与相关指标的影响

Effect of Ligustrazine Injection combined with Low Molecular Weight Heparin on Serum Levels of MMP-2, Adiponectin and 25-hydroxy Vitamin D in Patients with Early Onset Severe Eclampsia

目的:探讨川芎嗪注射液联合低分子肝素钠对早发型重度子痫患者血清MMP-2、脂联素及25-羟维生素D水平的影响。方法:选取我院治疗并符合纳入标准的早发型重度子痫孕妇48例,按随机、对照临床研究原则分对照组24例和治疗组24例,对照组采用硫酸镁常规治疗;治疗组在硫酸镁上采用川芎嗪注射液联合低分子肝素钠治疗,两组共持续治疗7 d,治疗前后抽取肘部静脉血5 m L,采用ELISA法测定血清PIGF、脂联素、瘦素、MMP-2、TIMP-2表达水平,采用放射免疫法测定血清25-OH-VD表达水平,观察并记录所有孕妇妊娠结局和并发症状况。结果:与治疗前比较,两组治疗后血清PIGF、25-OH-VD、脂联素表达水平升高,血清瘦素、MMP-2、TIMP-2表达水平降低(P<0.05);与对照组比较,治疗组治疗后血清PIGF、25-OH-VD、脂联素表达水平较高,血清瘦素、MMP-2、TIMP-2表达水平较低(P<0.05);对照组在收缩压、舒张压上高于治疗组,在终止妊娠孕周、新生儿出生体重上低于治疗组(P<0.05);两组间并发症发生率比较,差异无统计学意义(P>0.05)。结论:川芎嗪注射液联合低分子肝素钠有助于改善早发型重度子痫患者的妊娠结局,这与降低MMP-2和提高脂联素、25-羟维生素D水平关系密切。

Objective： To investigate the effects of lignstrazine injection combined with low molecular weight heparin on serum levels of MMP-2, adiponectin and 25-hydroxy vitamin D in patients with early onset severe eclampsia. Methods： The 48 patients with early onset severe severe eclampsia were randomly divided into 2 groups, 24 cases each. The control group was treated with Magnesium Sulfate, and the treatment group was given ligustrazine injection combined with low molecular weight heparin sodium besides the treatment of Magnesium Sulfate. The two groups were continuously treated for 7 days, the levels of serum PIGF, adiponectin, leptin, MMP-2 and TIMP-2 were measured by ELISA. The level of serum 25-OH-VD expression was measured by ra- dioimmunoassay, and the pregnancy outcome and complications of all pregnant women were observed and recorded. Results： Compared with before treatment, the levels of serum PIGF, 25-OH-VD and adiponcctin were increased in the two groups, and the levels of serum leptin, MMP-2 and TIMP-2 were decreased （P〈0.05）. Compared with the control group, the expression of serum PIGF, 25-OH-VD and adiponectin in the treatment group after treatment was higher, and the levels of serum leptin, MMP-2 and TIMP-2 were lower （P〈0.05）. The systolic blood pressure and diastolic blood pressure in the control group were higher than those in the treatment group, and the gestational age at the termination of pregnancy and neonatal birth weight were lower than those in the treatment group （P〈0.05）. There was no significant difference in the incidence of complications between the two groups （P〉0.05）. Conclusion： Ligustrazine injection combined with low molecular weight heparin can help to improve the pregnancy outcome in patients with early onset severe eclampsia, which is closely related to the reduction of MMP-2 and the improvement of adiponectin and 25- hydroxyvitamin D levels.
