超声引导下经胸壁段腋静脉置入中心静脉导管在295例患者中的应用

Ultrasound guided insertion of central venous catheter through axillary vein at chest wall: An application study of 295 patients

目的回顾分析经胸壁段腋静脉置入中心静脉导管的临床应用情况,为临床选择腋静脉置管提供参考。方法对295例超声引导下经胸壁段腋静脉置入中心静脉导管的病例资料进行回顾分析,评价指标包括置管时间、置管长度、一次性穿刺成功率、一次性送管成功率、置管总成功率、置管并发症发生情况、导管留置时间、留置期间并发症发生情况等。结果置管时间(6.93±3.35)min,置管长度(17.26±0.77)cm,一次性穿刺成功率92.2%,一次性送管成功率94.42%,置管总成功率97.29%,置管并发症发生率7.80%,导管留置时间(60.69±50.06)d,留置导管期间并发症发生率7.67%。结论实时超声引导下经胸壁段腋静脉置入中心静脉导管具有可行性,导管留置时间长、安全性高。

Objective To retrospectively analyze the clinical application of central venous catheterization via axillary vein at chest wall in order to provide references for clinical options of axillary vein eannulation. Methods The clinical data of 295 patients were retrospectively analyzed,who underwent central venous catheterization via axillary vein at chest wall guided by ultrasound. The indicators for evaluation included duration of catheterization, length of catheter,success rate of one-time insertion,success rate of one-time advancing,total success rate of catheterization,complications of catheterization,indwelling time of catheter,complications during indwelling,etc. Results Duration of catheterization was 6.93±3.35 min,length of catheter was 17.26±0.77 cm,success rate of one-time insertion was 92.20% ,success rate of one-time advancing was 94.42% ,total success rate of catheterization was 97.29%,the incidence of complications was 7.80%,indwelling time of catheter was 60.69±50.06 d,and the incidence of complications during indwelling was 7.67%. Conclusion The central venous catheterization via axillary vein at chest wall guided by ultrasound had feasibility,high security and long duration of catheter indwelling.
