益气化瘀方预防子宫内膜异位症术后复发的临床研究

Yiqi Huayu Recipe on Prevention of Postoperative Recurrence of Endometriosis

目的：探讨益气化瘀方预防子宫内膜异位症术后复发的临床疗效。方法：子宫内膜异位症患者86例，随机分为对照组和观察组，每组患者各43例。对照组患者给予米非司酮预防复发，观察组患者在米非司酮预防基础上加用自拟益气化瘀方预防复发。观察并比较两组患者妊娠率、复发率和不良反应率及治疗前后性激素水平、中医体征积分、痛经评分和CA125水平。结果：观察组患者妊娠率为83．72％高于对照组患者，复发率为2．33％低于对照组患者，差异有统计学意义（P〈0．05）。两组患者治疗前FSH、E2及LH水平比较，差异无统计学意义（P〉0．05）；两组患者治疗后FSH和LH水平较治疗前下降，E2水平较治疗前上升（P〈0．05）；观察组患者治疗后FSH和LH水平较对照组患者治疗后低，E2水平较对照组患者治疗后高（P〈0．05）。两组患者治疗前中医体征积分、痛经评分及CA125水平比较，差异无统计学意义（P〉0．05）；两组患者治疗后中医体征积分、痛经评分及CA125水平较治疗前下降（P〈0．05）；观察组患者治疗后中医体征积分、痛经评分及CA125水平较对照组患者治疗后低（P〈0．05）。观察组患者不良反应率为18．60％显著低于对照组患者，差异有统计学意义（P〈0．05）。结论：益气化瘀方治疗子宫内膜异位症能有效降低患者术后复发率和不良反应率，提高妊娠率和生活质量，调节患者体内性激素和CA125水平，值得推荐。

Objective： To investigate the clinical effect of Yiqi Huayu Recipe on preventing postoperative recurrence of endometriosis. Methods： Eighty-six patients with endometriosis treated were randomly divided into control group and observation group and each group had a total of 43 cases. Patients in the control group were given mifepristone to prevent recurrence,and the observation group was treated with mifepristone to prevent recurrence. The pregnancy rate,recurrence rate and adverse reaction rate of the two groups were observed and compared before and after treatment. The levels of sex hormones,the score of traditional Chinese medicine,dysmenorrhea score and CA125 level were observed and compared.Results： The pregnancy rate of the observation group was 83. 72%,higher than that of the control group. The recurrence rate of the observation group was 2. 33%,lower than that of the control group. The difference was statistically significant（ P 〉0. 05）. Two groups＇ FSH,E_2 and LH were not statistically significant before treatment（ P 〈0. 05）. The two groups＇ FSH and LH decreased compared after treatment and the level of E_2 was increased（ P 〈0. 05）. The observation group＇s FSH and LH levels were lower than those of the control group and the E_2 level was higher than that of the control group（ P 〈0. 05）. The TCM symptom integral of two groups,the dysmenorrhea score and the level of CA125 were not statistically significant（ P 〉0. 05）. After treatment,the two groups＇ TCM symptom integral,dysmenorrhea score and the level of CA125 decreased（ P 〈0. 05）. The observation group＇s TCM symptom integral,dysmenorrhea score and CA125 levels after treatment were lower than those of the control group after treatment（ P 〈0. 05）. The adverse reaction rate of the observation group was 18. 60%,which was significantly lower than that of the control group. The difference was statistically significant（ P 〈0. 05）. Conclusion： Yiqi Huayu Recipe can effectively reduce the recurrence rate and adverse reaction rate,improve the pregnancy rate and quality of life,adjust the levels of sex hormones and CA125 in patients with endometriosis,which is worthy of recommendation.
