寒喘祖帕颗粒联合布地奈德治疗缓解期小儿哮喘疗效及其对肺功能的影响

Clinical Effect of Hanchuan Zupa Granules with Budesonide in Children with Catabasis Bronchial Asthma at Reliving Stage and Its Influence on Lung Function

目的：探讨寒喘祖帕颗粒联合布地奈德治疗缓解期小儿哮喘疗效及其对患儿肺功能的影响。方法：86例缓解期小儿哮喘患儿,采用随机数字表法将其分为观察组和对照组,每组均为43例。所有患儿均进行纠正酸碱与电解质平衡等常规治疗,对照组同时给予布地奈德,2次/d,200μg/次。观察组在对照组基础上另给予寒喘祖帕颗粒治疗,1袋/次,2次/d,两组患儿的治疗时间均为1个月。治疗后,观察两组患儿临床疗效。治疗期间及治疗后,观察两组喘息、胸闷、咳嗽及肺部哮鸣音等症状的缓解时间。治疗前后,分别检测两组患儿肺功能指标（PEF、FEV1/FVC%、FEV1及FVC）,血气指标（PaO2、PaCO2及pH）,炎性因子（TNF-α、IL-1β、IL-4及hsCRP）。结果：治疗后,观察组总有效率为93.0%,明显高于对照组的72.1%（P〈0.05）。治疗期间,观察组喘息、胸闷、咳嗽及肺部哮鸣音等症状的缓解时间分别为（5.17±0.67）d、（3.87±0.35）d、（4.34±0.47）d及（7.21±0.79）d,均明显低于对照组（P〈0.05）。治疗后,观察组PEF、FEV1/FVC%、FEV1及FVC水平分别为（6.97±1.01）L/S、（55.18±5.06）%、（2.97±0.32）L及（5.57±0.53）L/S,均明显高于对照组（P〈0.05）。治疗后,观察组PaO2、PaCO2及pH值水平分别为（73.24±7.47）mmHg、（52.15±6.27）mmHg及（7.37±0.13）,改善程度均明显优于对照组（P〈0.05）。治疗后,观察组血清TNF-α、IL-1β、IL-4及hs-CRP水平均明显低于对照组（P〈0.05）。结论：寒喘祖帕颗粒联合布地奈德治疗缓解期小儿哮喘疗效较好,能够有效改善患儿肺部功能,缓解机体炎症反应,值得进行深入研究。

Objective： To explore the clinical effect of Hanchuan Zupa Granules with budesonide in children with catabasis bronchial asthma at the reliving stage and its influence on the lung function. Methods： Eighty-six children with bronchial asthma were randomly divided into control group（ 43 cases） and treatment group（ 43 cases）. The patients were treated by regular treatment,including corrected electrolyte balance,and the control group was treated with budesonide while the treatment group with Hanchuan Zupa Granules on the basis of control group. The treatment time was 6 months. The clinical effects in the 2 groups were compared after treatment. Before and after treatment,the levels of lung function indexes（ PEF,FEV1/FVC%,FEV1 and FVC）,blood gas index（ PaO2,PaCO2 and pH）,and inflammation factor（ TNF-α,IL-1β,IL-4 and hs-CRP） in the two groups were observed. During treatment,the relief time of symptoms such as wheezing,chest tightness,coughs and asthma in the lungs were observed. Results： The total effect rate was 93. 0% in treatment group,significantly higher than 72. 1% in control group（ P 〈0. 05）. The relief time of wheezing,chest tightness,coughs and asthma in the lungs were（ 5. 17 ± 0. 67） d,（ 3. 87 ± 0. 35） d,（ 4. 34 ± 0. 47） d and（ 7. 21 ± 0. 79） d,significantly lower than those of the control group（ P 〈0. 05）. After treatment,the levels of PEF,FEV1/FVC%,FEV1 and FVC were（ 6. 97 ± 1. 01） L/S,（ 55. 18 ± 5. 06） %,（ 2. 97 ± 0. 32） L and（ 5. 57 ± 0. 53） L/S,significantly higher than those of the control group（ P 〈0. 05）,and levels of PaO2,PaCO2 and pH were（ 73. 24 ±7. 47） mmHg,（ 52. 15 ± 6. 27） mmHg and 7. 37 ± 0. 13,significantly promoted than those of the control group（ P 0. 05）. After treatment,the levels of TNF-α,IL-1β,IL-4 and hs-CRP were significantly lower in treatment group than those of the control group（ P 〈0. 05）. Conclusion： Hanchuan Zupa Granules with budesonide has good effect on children with catabasis bronchial asthma at the reliving stage and can effectively improve the lung function and relieve the inflammation reactions,worthy of deeply studying.
