正常妊娠和妊娠期高血压疾病血管性血友病因子的表达与意义

The Expression and Significance of the von Willebrand Factor in Normal Pregnancy and Hypertensive Disorders Complicating Pregnancy

目的探讨血管性血友病因子(v WF)在不同孕周的正常妊娠妇女以及妊娠期高血压疾病患者中的表达与意义。方法采用免疫比浊法检测690例不同孕期正常妊娠妇女以及72例妊娠期高血压疾病患者(妊娠期高血压组20例,轻度子痫前期组27例,重度子痫前期组25例)外周血中v WF的水平。结果正常妊娠20~28周组孕妇的v WF表达高于妊娠12~20周组的孕妇,正常妊娠28~36周组及36周以上组的孕妇v WF表达高于妊娠12周之前组、12~20周组及20~28周组的孕妇v WF值,差异具有显著的统计学意义(P<0.01);妊娠期高血压组,轻度子痫前期组和重度子痫前期组对比正常妊娠组v WF的表达升高,重度子痫前期组对比妊娠期高血压组和轻度子痫前期组v WF的表达升高,差异有统计学意义(P<0.01)。结论正常妊娠妇女外周血中v WF的表达随着孕周的增大而升高;妊娠高血压疾病患者v WF水平显著高于正常妊娠妇女,而且表达水平随着病情加重而增高。v WF表达在正常妊娠中起重要作用,其高表达与妊娠期高血压疾病及病情严重程度有关。

Objective Study of the von Willebrand factor（vWF） expression and significance of normal pregnancy and hypertensive disorders complicating pregnancy （HDCP）. Metods Expression levels of vWF in 690 cases normal pregnancy and 72 cases HDCP （20 cases of gestational hypertension; 27 cases of mild preeclampsia;25 cases of severe preeclampsia）by immune turbidimetry. Results The vWF in normal pregnancy 20-28 weeks is more than 12-20 weeks;Normal pregnancy 28-36 weeks and more than 36 weeks compare with before 12 weeks,12-20 weeks and 20-28 weeks ,the expression of vWF in significantly increased （P〈O.01） ; Gestational hypertension, mild preeclampsia and severe preeclampsia compared with normal pregnancy group, the expression of vWF in significantly increased （P〈O.01） ;severe preeclampsia compared with gestational hypertension and mild preeclampsia, the expression of vWF in significantly increased （P 〈 0. 01 ）. Conclusion As the weeks of pregnancies increased, the expression levels of vWF was raised. The vWF levels of HDCP group is significantly increased than normal pregnancy group. The expression intersity of vWF increased with the degree of HDCP, which is associated with the occurrence and the degree.
