集束化护理干预对肿瘤患者经外周中心静脉置管并发症的影响

Effects of Cluster Nursing Intervention on Complications of Peripheral Central Venous Catheterization in Tumor Patients

目的探讨集束化护理干预对肿瘤患者经外周中心静脉置管(PICC)并发症的影响。方法通过对我科2013年5月到2015年7月肿瘤科就诊收治的行PICC的700例肿瘤患者资料做回顾性分析,根据护理方式的不同,分为干预组和对照组(各350例),比较并分析两组患者并发症发生情况及相关凝血主要指标(a PTT、PT、TT、Fbg、BPC)变化。结果干预组血栓形成、机械性静脉炎、导管阻塞发生率均明显低于对照组(P<0.05),两组感染、出血点、导管破损等并发症发生率比较无明显差异(P>0.05),干预组的a PTT、PT、TT指标明显上调,Fbg、BPC下调(P<0.05)。结论在行PICC的肿瘤患者中应用集束化护理干预,可减少患者置管前、置管过程中、置管后并发症发生的危险性。

Objective To investigate the effect of cluster nursing on the complications of peripheral central venous catheterization（PICC） in patients with cancer. Methods The clinical data of 90 patients PICC admitted to our hospital from May 2013 to July 2015 were retrospectively analyzed. The patients were divided into intervention group and control group according to the different nursing methods. Two groups of patients with complications and related coagulation changes in the main indicators （ aFIT, PT, TT, Fbg, BPC ） were analyzed and compared. Results The incidence of thrombosis, mechanical phlebitis and catheters in the intervention group were significantly lower than those in the control group, the difference was statistically significant （ P 〈 0.05 ）. No significant difference was detected in the incidence of complication between the two groups （P 〉 0.05 ）. The aFIT, PT and TF of the intervention group were significantly higher than those of the control group （ P 〈 0. 05 ）. The Fbg BPC indexes of the intervention group were significantly lower than those of the control group（P 〈0.05）. Conclusion The use of cluster care in patients with PICC can reduce the risk of complications before catheterization, in catheterization process and after catheterization.
