集束化护理对乳腺癌术后患者心理弹性及社会支持度及希望水平的影响

Effect of cluster nursing on the psychological elasticity,social support and hope level in patients with breast cancer after surgery

目的探讨集束化护理对乳腺癌术后患者心理弹性、社会支持度及希望水平的影响。方法选取2015年4月至2017年4月间山东省东营市第二人民医院收治的80例乳腺癌患者进行回顾性分析,将采用常规护理联合集束化护理的40例患者纳入观察组,采用常规护理40例患者纳入对照组,比较两组患者术后心理弹性评分、社会支持度评分及希望水平。结果两组患者护理后与护理前比较,坚韧、自强及乐观评分均升高,且观察组上述指标比对照组改善更显著,差异均有统计学意义(均P<0.05)。两组患者护理后与护理前比较,客观支持、主观支持及社会支持利用度评分均升高,且观察组上述指标比对照组改善更显著,差异均有统计学意义(均P<0.05)。两组患者护理后与护理前比较,希望总体、对现实和未来的积极态度(T)、采取积极的行动(P)及与他人保持亲密的关系(I)评分均升高,观察组上述指标比对照组改善更显著,差异均有统计学意义(均P<0.05)。结论在常规护理基础上加用集束化护理干预,能获得更好的心理弹性,社会支持度较高且希望水平较好。

Objective To explore and analyze the impact of cluster nursing on psychological elasticity,social support and hope level in patients with breast cancer after surgery. Methods Eighty patients with breast cancer treated at Second People＇s Hospital Of Dongying from April 2015 to April 2017 were selected and were divided into an observation group and a control group with 40 patients in each group. The observation group received cluster care on the basis of routine care and the control group was only given routine nursing care. Postoperative psychological elasticity score,social support and hope levels were compared between the two groups. Results Ater nursing,tenacity,self-renewal and optimism scores were increased compared with before nursing with the above scores higher in the observation group than in the control group（ all P〈 0. 05）. Objective support,subjective support and social support rates were improved in both groups with the observation group better than the control group（ all P〈 0. 05）. The positive attitude towards the reality and the future（ T）,taking positive actions（ P）,and keeping close relationship with others（ I） scores were improved in the two groups with the increase higher in the observation group than in the control group（ all P〈 0. 05）. Conclusion Cluster care on the basis of routine care can result in good psychological elasticity,high social support and good hope level.
