双氧水和复方黄柏液交替冲洗促进肛周坏死性筋膜炎术后创面愈合作用观察

Clinical Study on Wound Healing of Perianal Necrotizing Fasciitis by Chinese and Western Medicine

目的:研究运用双氧水和复方黄柏液交替隔日冲洗坏死性筋膜炎创面对术后创面的影响。方法:将12例患有肛周坏死性筋膜炎的患者按照随机的方式,分为研究组和对照组,每组6例。2组患者均采用相同的手术方式,研究组术后创面予以双氧水与复方黄柏液隔日交替冲洗,对照组创面只采用双氧水冲洗。密切观察2组患者术后1~15天创面水肿、愈合、患者疼痛及术后体温等情况。结果:研究组的患者疼痛评分显著低于对照组(P<0.05);研究组创面的水肿改善率高于对照组,分别为93%和79%;2组分别有0%和33%的患者出现发热症状;2组患者创面愈合时间分别为(34.1±4.5)d和(42.3±7.2)d,2组比较差异均有统计学意义(P<0.05)。结论:肛周坏死性筋膜炎术后创面运用过氧化氢与复方黄柏液交替隔日冲洗,对减轻术后创面水肿、疼痛、防治感染及加速术后创面愈合效果显著优于一般治疗,值得推广。

Objective： To study the effects of using hydrogen peroxide and compound phellodendron to alternately wash necrotic fasciitis on the wound surface after surgery. Methods： Twelve patients with perianal necrotizing fasciitis were randomized into study and control groups, 6 in each group. All the patients in the two groups used the same surgical method. The wounds of the study group were alternately flushed with hydrogen peroxide and compound phellodendron alternately every other day. In the control group, the wounds were only flushed with hydrogen peroxide. Close observation of wound edema, healing, patient pain, and postoperative body temperature occurred in 1 to 15 days after operation in the 2 groups. Results： The pain scores of the study group were significantly lower than those of the control group（P〈0.05）. The edema improvement rate in the study group was higher than that in the control group, which was93% and 79%, respectively. Fever symptoms occurred in 0% and 33% of patients in the 2 groups, respectively. The wound healing time of the two groups was（34.1±4.5）d and（42.3±7.2）d, respectively. There was significant difference between the two groups（P〈0.05）. Conclusion： Perioperation of perianal necrotizing fasciitis can be performed alternately with hydrogen peroxide and compound phellodendron alternately every other day. It is superior to general treatment in reducing wound edema, pain, infection control and postoperative wound healing. It is worth promoting.
