血浆NT-proBNP水平评估早期慢性阻塞性肺疾病对左心功能的临床研究

摘要 目的探讨血浆N-末端脑钠肽前体(NT-proBNP)水平对早期慢性阻塞性肺疾病(慢阻肺)的左心功能评估价值。方法选取50例早期慢阻肺患者作为试验组,另取40例同期健康体检者为对照组。试验组在常规治疗前后检测血浆NT-proBNP水平,对照组直接检测血清NT-proBNP水平。观察试验组治疗前与对照组血清NT-proBNP、左心室射血分数(LVEF)、第1秒用力呼气容积占预计值百分比(FEV1%)、第1秒用力呼气容积占用力肺活量的百分比(FEV1/FVC)以及试验组治疗前后血清NT-proBNP水平。结果试验组NT-proBNP水平(417.40±258.41)pg/ml高于对照组的(76.69±10.94)pg/ml,差异有统计学意义(P<0.05);试验组FEV1%、FEV1/FVC、LVEF略低于对照组,但差异无统计学意义(P>0.05);试验组治疗前的NT-proBNP(417.40±258.41)pg/ml高于治疗后的(134.45±208.43)pg/ml,差异有统计学意义(P<0.05)。结论早期慢阻肺患者已出现血清NT-proBNP升高,左心功能已受到影响。 Objective To investigate the effect of N-terminal pro-brain natriuretic peptide(NT-proBNP) for the left ventricle function at early stage of chronic obstructive pulmonary disease(COPD). Methods 50 patients with early COPD were selected as experimental group, 40 normal heathy subjects at the same period were selected as control group. NT-proBNP levels of patients in the observation group were measured before and after treatment. The serum NT-proBNP levels in the control group were also tested. The NT-proBNP levels, left ventricular ejection fraction(LVEF), the first second forced expiratory volume percentage of expected value(FEV1%) and ratio of percentage of forced expiratory volume in first second and forced vital capacity(FEV1/FVC%) of experimental group before treatment and control group were observed, and the serum levels of NT-proBNP in experimental group before and after treatment were compared. Results The serum NT-proBNP level of the experimental group was(417.40±258.41) pg/ml, which was significantly higher than(76.69±10.94) pg/ml of the control group, the difference was statistically significant(P〈0.05). The FEV1%, FEV1/FVC and LVEF of the experimental group were slightly lower than the control group, but there were no statistically significant differences(P〉0.05). In the experimental group, the serum NT-proBNP level before treatment was(417.40±258.41) pg/ml, which was significantly higher than(134.45±208.43) pg/ml after treatment, the difference was statistically significant(P〈0.05). Conclusion At early stage of COPD, the serum NT-proBNP of patients have increased and LVEF have been affected.
