弯腰挺立手法治疗腰骶关节紊乱的规范化研究

Standardization Research on Wanyaotingli Manipulation Treating for Lumbar Sacral Joint Disorder

目的:分析不同操作者使用弯腰挺立手法治疗腰骶关节紊乱的差异性和相似度,总结该手法的运动学操作特征及要点。方法:随机将60例健康志愿者平均分到2名有弯腰挺立手法临床操作经验的中医师(记为A医师和B医师),通过在志愿者和操作者身上特定位置安置标记点进行标记,用运动捕捉系统对弯腰挺立法手法操作过程中操作者及志愿者的全身运动状态进行动态描记,获取该手法操作过程中操作者及志愿者的运动学参数。结果:弯腰挺立手法过程A和B组操作者、志愿者的前屈、后伸角度等各数据,差异无统计学意义(P>0.05)。两操作者总体前屈平均角度35.73°±4.62°,后伸平均角度28.54°±4.63°,髋部平均作用时间(495.08±118.38)ms,平均位移(291.20±62.03)mm;两组志愿者前屈平均角度89.54°±7.36°,后伸平均角度29.17°±5.05°,抛出平均高度(95.72±34.19)mm,平均水平位移(547.80±85.67)mm,抛出平均角度34.31°±10.87°。结论:不同操作者使用弯腰挺立手法治疗腰骶关节紊乱没有明显差异,弯腰挺立手法对于操作者和受试者的前屈、后伸角度等运动学参数都有一定的标准。

Objective：To analyze the difference and similarity of manipulators using Wanyaotingli manipulation to treatment of lumbar sacral joint disorder and summarize the kinematical operation characteristics and key points of this manipulation.Methods：Assign 60 heathy volunteers to 2 doctors of traditional Chinese medicine（TCM）with clinical experience of Wanyaotingli manipulation on average and mark the volunteers and manipulators on special location with markers,and then obtain the kinematics parameter by using motion capture system during the process of manipulation.Results：During the process of manipulation,there was no statistical significance between the manipulators＇ and the volunteers＇ anteflexion angle and back-extending angle and so on（all P〈0.05）.The manipulators＇ average anteflexion angle is 35.73°±4.62°,the average angle of back-extending was 28.54°±4.63°,the hip＇s average action time was（495.08±118.38）ms,the average horizontal displacement was（291.20±62.03）mm.The volunteers＇ average anteflexion angle was 89.54°±7.36°,the average angle of back-extending was 29.17°±5.05°,the average vertical displacement was（95.72±34.19）mm,the average horizontal displacement was（547.80±85.67）mm,and the average angle of throwing out was 34.31°±10.87°.Conclusion：There is no significant difference between the two manipulators using Wanyaotingli manipulation to treat lumbar sacral joint disorder,and there are certain standards on the volunteers and manipulators＇ kinematics parameters such as anteflexion and rear protraction of this manipulation.
