高压氧治疗原发性三叉神经痛的临床研究

Clinical Study of Hyperbaric Oxygen in the Treatment of Primary Trigeminal Neuralgia

目的:探究原发性三叉神经痛应用高压氧(HBO)治疗的临床效果。方法:选取2017年1-7月本院收治的原发性三叉神经痛患者60例。按照随机数字表法将其分为对照组与研究组,各30例。对照组给予卡马西平口服治疗,研究组在对照组基础上增加HBO治疗。比较两组的短期疗效[治疗期间不良反应、治疗结束后的视觉模拟疼痛评分(VAS)、治疗效果]、治疗前后不同时间的VAS评分、治疗后不同时间点对治疗的总体感觉、治疗前后卡马西平的口服剂量。结果:研究组治疗结束后VAS评分低于对照组(P<0.05),临床总有效率高于对照组(P<0.05),两组治疗期间不良反应发生率比较差异无统计学意义(P>0.05);治疗后1、2、3、6个月,研究组VAS评分均低于对照组(P<0.05);治疗结束时及治疗后6个月,研究组治疗感觉有效率为76.67%、80.00%,均高于对照组的43.33%、36.67%,比较差异均有统计学意义(P<0.05);治疗后1、2、6个月,研究组口服药物剂量均小于对照组,比较差异均有统计学意义(P<0.05)。结论:HBO治疗原发性三叉神经痛可改善患者疼痛、提升临床效果、改善患者对治疗的总体感觉。

Objective：To explore the clinical effect of hyperbaric oxygen（HBO） therapy for primary trigeminal neuralgia.Method：A total of 60 patients with primary trigeminal neuralgia admitted in our hospital from January to July 2017 were selected.According to the random number table method,they were divided into control group and study group,30 cases in each group.The control group was given Carbamazepine oral treatment,and study group was given HBO treatment on the basis of control group.The short-term effects[adverse effects during treatment,visual analogue pain score（VAS） after treatment,treatment effect],VAS score at different time before and after treatment,overall feeling of the treatment at different time points after treatment and the dose of oral carbamazepine before and after treatment between two groups were compared.Result：After treatment,the VAS score of study group was lower than that of control group,and the total effective rate was higher than that of control group,the differences were statistically significant（P〈0.05）.The incidence of adverse reactions during the treatment of two groups were compared,the difference was not statistically significant（P〈0.05）.After treatment 1,2,3 and 6 months,the VAS score of study group were lower than those of control group（P〈0.05）.After treatment and after treatment 6 months,the effective rate of treatment feels in study group was 76.67% and 80.00%,which were all higher than 43.33% and 36.67% of control group,the differences were statistically significant（P〈0.05）.After treatment 1,2 and 6 months,the dosage of oral medication in study group were less than those of control group,the differences were statistically significant（P〈0.05）.Conclusion：The treatment of primary trigeminal neuralgia by HBO can improve the patients＇ pain,improve the clinical effect,and improve the overall feeling of the patient on the treatment.
