大容量全肺灌洗术中持续心电监护对手术安全性评估的意义

Significance of Continuous ECG Monitoring in the Evaluation of Surgical Safety in Large Volume Whole Lung Lavage

目的:探讨大容量全肺灌洗术中持续心电监护对手术安全性评估的意义。方法:选取2016年6月-2017年12月本院收治的尘肺患者86例。依据术中管理方法将其分为持续心电监护组(n=43)和常规心电监护组(n=43)。对两组患者的肺功能、心脏并发症发生情况进行统计分析。结果:手术后,两组FEV_1、FVC、FEV_1/FVC均高于手术前,且持续心电监护组均高于常规心电监护组,比较差异均有统计学意义(P<0.05);持续心电监护组并发症总发生率为4.7%(2/43),显著低于常规心电监护组的30.2%(13/43),比较差异有统计学意义(X^2=9.771,P<0.05)。结论:大容量全肺灌洗术中持续心电监护较常规心电监护更能有效提升手术安全性。

Objective：To explore the significance of continuous ECG monitoring in the evaluation of surgical safety in large volume whole lung lavage.Method：A total of 86 patients with pneumoconiosis from June 2016 to December 2017 in our hospital were selected.According to the intraoperative management,they were divided into continuous ECG monitoring group（n=43） and conventional ECG group（n=43）.The pulmonary function and incidence of cardiac complications in two groups were statistically analyzed.Result：After operation,the FEV1,FVC and FEV1/FVC in two groups were all higher than those of before operation,and continuous ECG monitoring group were all higher than those of conventional ECG monitoring group,the differences were statistically significant（P〈0.05）.The total incidence of complications in continuous ECG monitoring group was 4.7%（2/43）,which was significantly lower than 30.2%（13/43） of conventional ECG monitoring group,the difference was statistically significant（P〈0.05）.Conclusion：Continuous ECG monitoring in large volume whole lung lavage can more effectively improve the safety of operation than conventional ECG monitoring.
