髓芯减压+同种异体骨打压植骨治疗早期股骨头坏死的疗效观察

Treatment of early osteonecrosis of femoral head with core decompression plus allograft bone graft

目的分析早期股骨头坏死患者采用髓芯减压联合同种异体骨打压植骨方式进行治疗的临床效果。方法抽取以往在本院接受手术治疗的78例早期股骨头坏死患者,将其以随机分组的形式分成对照组和治疗组,每组39例。对照组采用股骨头颈部开窗打压植骨术进行治疗;治疗组采用髓芯减压联合同种异体骨打压植骨术进行治疗。比较两组在手术治疗前后髋关节功能Harris评分和疼痛程度VAS评分的改善幅度、早期股骨头坏死手术治疗效果。结果治疗组患者在手术治疗前后髋关节功能Harris评分和疼痛程度VAS评分的改善幅度大于对照组(P<0.05);早期股骨头坏死手术治疗总及格率水平达到92.3%,高于对照组的71.8%,组间差异具有统计学意义(P<0.05)。结论早期股骨头坏死患者采用髓芯减压联合同种异体骨打压植骨方式进行治疗,可以迅速改善髋关节功能,减轻疼痛,使治疗效果提升。

Objective To analyze the clinical effect of the treatment of early femoral head necrosis with core decompression combined with allograft bone pressing and bone grafting. Methods 78 patients with early stage osteonecrosis of the femoral head underwent surgical treatment in our hospital. They were randomly divided into control group and treatment group, 39 cases in each group. The control group was treated with fenestration of the femoral head and neck for the treatment of bone graft, and the treatment group was treated with core decompression combined with allograft bone grafting. The improvement of the Harris score of the hip joint function and the VAS score of the pain degree were compared between the two groups before and after the operation, and the effect of the operation on the early femoral head necrosis was compared. Results The patients in the surgery before and after treatment, the Harris hip score and pain VAS scores improved significantly higher than that in the control group（P〈0.05）;early treatment of osteonecrosis of femoral surgery total pass rate level reached 92.3%, higher than 71.8% in the control group, significant difference between groups（P〈0.05）. Conclusion The treatment of early stage osteonecrosis of the femoral head with core decompression and allograft bone graft can rapidly improve the function of the hip joint, relieve pain and improve the therapeutic effect.
