经伤椎置钉单椎间椎弓根螺钉复位固定治疗创伤性B型胸腰椎骨折的效果观察

Effect Observation of Injured Vertebral Nailing Single Interbody Pedicle Screw Reduction Fixation on Treatment of Traumatic Type B Thoracolumbar Fracture

目的:观察经伤椎置钉单椎间椎弓根螺钉复位固定治疗创伤性B型胸腰椎骨折的效果。方法:随机选定2015年9月-2017年9月本院收治的创伤性B型胸腰椎骨折患者110例,以随机数字表法分为试验组和对照组,每组55例。对照组采用跨伤椎短节段固定术治疗,试验组采用经伤椎置钉单椎间椎弓根螺钉复位固定术治疗。比较两组患者的临床疗效、手术情况、VAS评分、Oswestry功能障碍指数(ODI)评分、并发症发生情况。结果:试验组的总有效率高于对照组,术中出血量少于对照组,治疗后的VAS评分、ODI评分、并发症发生率均低于对照组,手术时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。结论:经伤椎置钉单椎间椎弓根螺钉复位固定术可有效减轻创伤性B型胸腰椎骨折患者的疼痛和功能障碍,缩短了治疗时间,降低了并发症发生率,安全可行,值得借鉴。

Objective：To observe the effect of injured vertebral nailing single interbody pedicle screw reduction fixation on treatment of traumatic type B thoracolumbar fracture.Method：A total of 110 patients with traumatic type B thoracolumbar fractures treated in our hospital from September 2015 to September 2017 were selected,according to the random number table method,the patients were divided into experimental group and control group,55 cases in each group.The control group was treated with short-segment transcranial vertebral fixation,and the experimental group was treated with a single interbody pedicle screw fixation.The clinical effect,operation condition,VAS score and ODI score,and the occurrence of complications between the two groups were compared.Result：The total effective rate of the experimental group was higher than that of the control group,the amount of intraoperative bleeding was less than that of the control group,the VAS score and the incidence of complications were lower than those in the control group,the operation time and the hospital stay were shorter than those in the control group,the differences were statistically significant（P〈0.05）.Conclusion：The combined pedicle screw fixation and fixation with pedicle screw fixation can effectively reduce the pain and dysfunction in patients with traumatic type B thoracolumbar fractures,shorten the treatment time,reduce the incidence of complications,safe and feasible,it is worth reference.
