摘要
目的研究无黏弹剂辅助的人工晶体植入术的临床效果。方法选取笔者所在医院年龄相关性白内障45例患者,随机分为两组:无黏弹剂组24例30只眼,常规黏弹剂组21例30只眼。术前常规进行全身及眼科专科检查,两组患者分别采用无黏弹剂辅助的人工晶体植入术和常规黏弹剂辅助的人工晶体植入术。分别在术后1周、1个月、6个月观察并比较两组患者的角膜内皮细胞计数、角膜内皮细胞六边形细胞百分率及中央角膜厚度。结果两组术前角膜内皮细胞计数、角膜六边形细胞百分率及中央角膜厚度比较,差异均无统计学意义(P>0.05)。术后1周、1个月、6个月两组的角膜内皮细胞计数及角膜六边形细胞百分率均较术前减少,但在各个时间点无黏弹剂组角膜内皮细胞计数及角膜六边形细胞百分率的减少程度均比常规黏弹剂组小,差异具有统计学意义(P<0.05)。术后1周、1个月、6个月两组的中央角膜厚度均较术前增加,两组中央角膜厚度变化幅度相比差异没有统计学意义(P>0.05)。结论无黏弹剂辅助的人工晶体植入术缩短手术时间,减少并发症,对角膜内皮影响更小,角膜安全性更高。
Objective To observe the clinical effect of intra ocular lens implantation without viscoelastic.Methods The patients with age-related cataract(n=45,60 eyes) were randomly divided into 2 groups:group A(24 patients,30 eyes) underwent phacoemulsification and intraocular lens implantation without viscoelastic,and group B(21 patients,30 eyes) underwent phacoemulsification and intraocular lens implantation with viscoelastic.The corneal endothelial cell count,percentage of hexagonal cells and central corneal thickness were observed at 1 week,1 month and 6 months after surgery. Results There was no significant difference in basic condition between 2 groups(P〈0.05). All corneal endothelial cell count and percentage of hexagonal cells from patients after surgery were less compared with preoperative. There were significant differences in endothelial cell counting loss and decrease of percentage of hexagonal cells between 2 groups at 1 week,1 month and 6 months after surgery(all,P〈0.05). The central cornea thickness in both groups both increased after surgery. There were no significant differences in central cornea thickness between 2 groups at 1 week,1 month and 6 months after operation(all,P〈0.05). Conclusion Intraocular lens implantation without viscoelastic has shorten time of operation and can reduce complications,less impact on corneal endothelial cells with higher corneal security.
