温肾益脾方联合阿卡波糖治疗早期糖尿病肾病的疗效观察及对糖代谢和肾功能的影响

Curative Observation of Using Wenshen Yipi Recipe Combined with Acarbose in the Treatment of Early Diabetic Nephropathy and Its Effect on Glucose Metabolism and Renal Function

目的:观察温脾益肾方联合阿卡波糖片治疗早期糖尿病肾病的临床疗效,以及对糖代谢及肾功能的影响。方法:选取60例早期糖尿病肾病并且辨证为脾肾气虚夹血瘀患者,随机分为对照组和观察组,每组30例。2组在常规治疗基础上,对照组给予口服阿卡波糖片+结沙坦胶囊治疗,观察组给予口服阿卡波糖片+温肾益脾方治疗,8周为1疗程,连续治疗2疗程。检测2组患者治疗前后血糖指标、蛋白尿相关指标及肾功能指标,并进行统计分析;观察并记录治疗前后中医证候积分;同时比较2组的临床疗效。结果:2组治疗后FPG、2h PG及Hb A1c均显著降低(P<0.05),但2组间比较没有显著差异(P>0.05);治疗后2组的24h UTP、U-β2MG、NAG、m Alb及UAER均明显降低(P<0.05),并且观察组较对照组下降更显著(P<0.05);治疗后2组的Scr、BUN、Cys-C及Hcy均明显降低(P<0.05),并且观察组Scr、Cys-C及Hcy较对照组下降更显著(P<0.05)。2组患者治疗后中医症候积分均显著降低(P<0.05),并且观察组治疗后的除纳呆、口唇紫暗外其余各项评分较对照组低(P<0.05)。观察组总有效率90.0%,对照组总有效率76.7%,差异显著(P<0.05)。结论:温脾益肾方联合阿卡波糖片治疗早期糖尿病肾病可以有效调节糖代谢,提高肾功能,疗效确切。

Objective： To observe the clinical effect of using Wenpi Yishen Decoction combined with Acarbose Tablet in the treatment of early diabetic nephropathy, and the effects on glucose metabolism and renal function. Methods ： 60cases of early dia- betic nephropathy and syndrome of spleen and kidney deficiency with blood stasis were randomly divided into controlled group and observation group, each group of 30cases. 2group on the basis of routine treatment, the controlled group was treated with Acar- bose Tablet＋Jiesha Tanzania Capsule treatment, the observation group was given oral Acarbose Tablet＋ Wenshen Yipi Recipe treatment, 8weeks as I course of treatment, continuous treatment of 2courses. The detection of 2groups before and after treatment of blood glucose index, proteinuria and renal function index, and the statistical analysis ; Observed and recorded before and after treatment of TCM syndrome integral; And compared the clinical efficacy of the 2groups. Results： The 2groups after treatment of FPG, 2hPG and HbAlc were significantly decreased （P〈0. 05）, but there was no significant difference between the 2groups （P 〉0.05 ） ; 24hUTP 2group after treatment, U-beta 2MG, NAG, mAlb and UAER were significantly decreased （ P〈0. 05 ）, and the observation group decreased significantly than the controlled group （P〈0.05） ; Scr, the 2groups after treatment of BUN, Cys -C and Hcy were significantly decreased （P〈0.05）, and the concept of comparing with the controlled group, the scores of TCM symptom scores in the observation group were significantly lower than those in the controlled group （P〈0. 05, P〈0.05 ）. The oth- er scores of the observation group were lower than that of the controlled group （P〈0. 05 ）. The total effective rate of the observa- tion group was 90. 00. The total effective rate of the controlled group was 76. 7%. The difference was significant （ P〈0. 05）. Conclusion： The combination of Wenpi Yishen Recipe and Acarbose Tablet is used in the treatment of early diabetic nephropathy can effectively regulate glucose metabolism and improve renal function.
