应用子宫内膜采集器行液基细胞学检查对子宫内膜癌的诊断价值分析

Diagnostic value of liquid based cytology using endometrial collector in patients with endometrial cancer

目的探讨应用子宫内膜采集器行液基细胞学检查对子宫内膜癌的诊断价值。方法选取行子宫内膜检查的子宫内膜癌高危患者89例,应用子宫内膜采集器行液基细胞学检查,同时行诊断性刮宫组织学检查。结果经诊断性刮宫组织学检查确诊的子宫内膜癌患者有11例,非子宫内膜癌患者有78例;子宫内膜采集器与诊断性刮宫的取材满意度比较,差异无统计学意义(P﹥0.05);子宫内膜采集器的采集时间短于诊断性刮宫,出血量和VAS评分均低于诊断性刮宫(P﹤0.05);液基细胞学检查诊断子宫内膜癌的灵敏度、特异度、阳性预测值和阴性预测值分别为72.73%、96.15%、72.73%和96.15%。结论应用子宫内膜采集器行液基细胞学检查对子宫内膜癌有较高的诊断价值,且使用子宫内膜采集器具有出血量少、疼痛轻的优点。

Objective To investigate the diagnostic value of liquid based cytology using endometrial collector in endometrial cancer. Method Eighty-nine patients with high risk of endometrial cancer were selected, the endometrial collector was used for liquid based cytology, and all patients underwent histological examination of diagnostic curettage as well. Re Result By diagnostic curettage, 11 cases of endometrial cancer were diagnosed and 78 cases were excluded. There was no significant difference in draw materials satisfaction between endometrial collector and diagnostic curettage（P〉0.05）. The acquisition time of endometrial collector was shorter than that of diagnostic curettage, the blood loss and VAS score of endometrial collector were less than those of diagnostic curettage（P〈0.05）. The sensitivity, specificity, positive predictive value and negative predictive value of liquid based cytology in diagnosis of endometrial cancer were 72.73%, 96.15%,72.73% and 96.15%, respectively. Conclusion Using endometrial collector for liquid based cytology in diagnosis of endometrial cancer has higher diagnostic value, and endometrial collector has the advantages of less bleeding and less pain.
