微型钢板与克氏针在急诊治疗闭合性近节指骨骨折的临床疗效分析

Analysis of clinical efficacy of mini-plate and kirschner wire in emergency treatment of closed proximal phalanx fractures

目的比较微型钢板与克氏针在急诊治疗闭合性近节指骨骨折的临床疗效。方法 2010年1月-2016年12月收治并行急诊手术修复闭合性近节指骨骨折68例,其中克氏针内固定组38例,微型钢板内固定组30例,术后随访2～12个月,平均5.6个月。采用美国手外科协会评分(TAFS)评价术后掌指关节及指间关节活动功能。对近节指骨骨折愈合时间、功能恢复情况及感染率进行统计及分析。结果微型钢板内固定组骨折愈合时间平均为4.6周,优良率为93.33%,术后无1例发生感染;克氏针内固定组骨折愈合时间平均为5.7周,优良率为86.84%,术后感染率7.89%。结论微型钢板内固定在术后骨折愈合时间、关节活动功能及感染率等均较克氏针内固定具有优势。

Objective To compare the clinical efficacy of mini-plate and Kirschner wire in emergency treatment of closed proximal phalanx fracture. Methods Sixty-eight cases of closed proximal phalanx fracture were treated by emergency surgical repair in Leizhou People＇s Hospital, Zhanjiang City, Guangdong province,from January 2010 to December 2016, 38 cases of Kirschner were in internal fixation group, 30 cases of mini-plate were in internal fixation group, patients were followed up for 2 - 12 months, an average of 5.6 months. The American Association of hand surgery （TAFS） score were used to evaluate the metacarpal phalangeal joint and the interphalangeal joint function. Statistics and analysis of the healing time, functional recovery and infection rate of proximal phalanx fractures were performed. Results The average fracture healing time of the mini-plate internal fixation group was 4.6 weeks, the excellence rate of is 93.33%, no infection occurred after the operation; the average fracture healing time of the Kirschner＇s internal fixation group was 5.7 weeks, the excellence rate is 86.84%, the postoperative infection rate was 7.89%. Conclusion Compared with Kirschner＇s internal fixation group, the fracture healing time, joint activity and infection rate of the internal fixation of mini-plate were superior to that of the Kirschner wire fixation group.
