2型糖尿病低血糖经历患者生活质量现状及影响因素研究

Influencing factors of quality of life in type 2 diabetes patients with hypoglycemia experience

目的调查有低血糖经历的2型糖尿病患者生活质量及其影响因素,为开展临床糖尿病教育工作提供依据。方法以多中心便利抽样法选取过去6个月内有低血糖经历的392例2型糖尿病患者,采用一般资料问卷、中文版低血糖恐惧调查—忧虑量表(Chinese version Hypoglycemia Fear Survey-Worry Scale,CHFSII-WS)、低血糖恐惧—行为量表、欧洲五维健康量表(the EuroQol-5 Dimensions Questionnaire,EQ-5D)对患者进行调查,并探索低血糖经历对生活质量的影响。结果健康指数量表(EuroQol-5 Dimensions index,EQ-5D index)得分为0.443~0.848分,0.785(0.705,0.848)分,视觉模拟量表(EuroQol-Visual Analague Scale,EQ-VAS)得分为10~100分,70(60,80)分。性别(女性)、周围神经病变、糖尿病足、每周运动时间(0 min)、因进食量过少而低血糖的经历、低血糖恐惧—忧虑为2型糖尿病低血糖经历患者生存质量的影响因素(P<0.05);并发症数量(>3个)、每日胰岛素注射次数(3~4次)、低血糖恐惧—行为、低血糖恐惧—忧虑为患者生存质量的影响因素(P<0.05)。结论医护人员更应关注女性、未进行运动、有并发症、胰岛素注射次数频繁、具有低血糖恐惧的患者。

Objective To investigate the influencing factors of quality of life in type 2 diabetes patients with hypoglycemia experience. Methods Through multi-center convenience sampling,totally 392 type 2 diabetes patients who had experienced hypoglycemia within 6 months were recruited. Questionnaires of Chinese version Hypoglycemia Fear Survey-Worry Scale,the Hypoglycemia Fear-Behavior scale,and the EuroQol-5 Dimensions questionnaire（EQ- 5D）were used to explore the potential influencing factors of quality of life in type 2 diabetes patients. Results The score range of EQ-5D index was from 0.443 to 0.848 and the median was 0.785 （0.705,0.848）. The score range of EQ-VAS was from 10 to 100 with the median of 70 （60,80）. Gender （female）,peripheral neuropathy,diabetic foot,weekly exercise time （Omin）,experience of hypoglycemia due to low intake of food,hypoglycemic fearworry were the influencing factors of EQ-5D index （P〈0.05）. Number of complications （〉3）,daily insulin injections （3-4 times）,hypoglycemic fear-behavior,hypoglycemic fear-worry were the influencing factors of EQ-VAS （P〈0.05）. Conclusion Health professionals should pay more attention to these type 2 diabetes patients who are female,without exercise,with complications,with frequent insulin injections and with hypoglycemic fear.
