间接免疫荧光法与被动凝集法对儿童肺炎支原体感染的诊断价值比较 被引量:4

摘要 目的:探讨间接免疫荧光法(Indirect immunofluorescent assav,IFA)与被动凝集法(Particle Agglutination Test,PAT)对初诊儿童肺炎支原体(myco—plasma pneumonia,MP)感染的诊断价值差异。方法:选取2016年3月-8月佛山市南海经济开发区人民医院儿科初诊患者,通过病史、症状和体征拟诊为疑似MP感染患者,采用诊断金标准将疑似患者(研究对象)确诊分为MP感染组(9(3例)和非MP感染组(121例)。所有研究对象均采用IFA法检测抗MP—IgM抗体,用PAT法检测抗MP总抗体,采用循证检验医学的方法统计两种方法的诊断价值性能指标。结果:IFA法和PAT法诊断儿童MP感染灵敏度分别为77.8%和66.7%,采用独立标本四格表xz检验(χ^2=1.91),表明两种方法的诊断灵敏度差异没有统计学意义(P〉0.05)。IFA和PAT诊断特异性分别为94.2%和66.9%,采用校正χ2检验(χ^2=6.95),表明两种方法的诊断特异性存在非常显著性差异(P〈0.01)。结论:诊断初诊儿童肺炎支原体感染,IFA法与PAT法比较诊断灵敏度无显著差异,诊断特异性优于PAT法. Objective: To explore the diagnostic value differences between Indirect Immunofluorescence Assay (IFA) and Particle AgglutinatiOn Test (PAT) in Children Pneumonia Mycoplasma (PM) infection. Methods: Serum samples of 90 patients with MP and 121 none-MP infection patients were detected. The IgM antibody was measured by IFA and the total antibody was measured by PAT. The patients and control group were newly diagnosed at Foshan Nanhai Economic Development Zone People's Hospital from March to August, 2014. Evidence-Based Laboratory Medicine was used to evaluate the diagnostic methods. Results: The diagnostic sensitivity oflFA and PAT were 77.8% and 73.3% respectively, and there was no statistical significance in χ^2 test (χ^2=1.42〈3.84, P〉0.05) . The diagnostic specificity oflFA and PAT were 94.2% and 66.9% respectively, indicating that there was a significant difference in the diagnostic specificity of the two methods (P〈0.01). Conclusion: The diagnostic specificity oflFA is better than PAT and there were no differences in the diagnostic sensitivity between oflFA and PAT.
