咬合重建固定义齿修复术治疗牙齿重度磨损并发牙列缺损的临床效果

Clinical effect of occlusal reconstruction and fixed denture restoration in treating severe tooth wear and dentition defect

目的探讨咬合重建固定义齿修复术治疗牙齿重度磨损并发牙列缺损的临床效果。方法选取攀枝花市中心医院2013年6月—2015年2月收治的牙齿重度磨损合并牙列缺损患者214例,按照随机数字表法分为对照组和研究组,每组107例。对照组患者实施牙合垫式可摘局部义齿修复术,研究组患者实施咬合重建固定义齿修复术。比较两组患者治疗前及治疗后1个月、1年咀嚼率、治疗前后颞下颌功能及治疗满意度。结果治疗前,两组患者咀嚼率比较,差异无统计学意义(P>0.05);治疗后1个月、1年,研究组患者咀嚼率高于对照组(P<0.05)。两组患者治疗1个月、1年咀嚼率高于治疗前(P<0.05)。治疗前,两组患者颞下颌功能比较,差异无统计学意义(P>0.05);治疗后,研究组患者颞下颌功能优于对照组(P<0.05)。研究组患者治疗满意度优于对照组(P<0.05)。结论咬合重建实施固定义齿修复术治疗牙齿重度磨损并发牙列缺损的临床效果确切,可有效改善患者颞下颌功能,提高患者治疗满意度。

Objective To evaluate the clinical effect of occlusal reconstruction and fixed denture restoration in treating severe tooth wear and dentition defect.Methods A total of 214 cases of severe tooth wear and dentition defect patients were selected from June 2013 to February 2015 in Panzhihua Central Hospital, which were divided into control group and study group according to random number table, 107 cases in each group.The control group was given occlusal pad type removable partial denture restoration, study group was given occlusal reconstruction and fixed denture restoration.The masticatory rate before treatment and after treatment of 1 month, 1 year, temporomandibular function before and after treatment, and treatment satisfaction were compared between the two groups.Results Before treatment, there was no significant difference of masticatory rate was found between the two groups （ P〉0.05 ） ; after treatment of 1 month, I year, the masticatory rate of study group was higher than control group （P〈0.05） .And after treatment of 1 month, 1 year, both groups of masticatory rate were higher than prior treatment （ P〈0.05 ） .Before treatment, there was no significant difference of temporomandibular function between the two groups （ P〉0.05 ） ; after treatment, the temporomandibular function of study group was better than control group （ P〈0.05 ） . The study group of treatment satisfaction was better than control group （P〈0.05） .Conclusion Occlusal reconstruction and fixed denture restoration has an exactly clinical effect in treating severe tooth wear and dentition defect, it can effectively improve the temporomandibular function of patients, promote patients＇ s satisfaction.
