超声联合弹性成像在鉴别非哺乳期乳腺炎和乳腺癌中的诊断价值 被引量:9

摘要 目的探讨超声联合弹性成像在鉴别非哺乳期乳腺炎和乳腺癌中的诊断价值。方法选择2016年1月—2017年3月天津市北辰医院接受治疗的患有乳腺病灶的患者102例作为研究对象,根据术后病理结果将其分为良性组和恶性组,应用超声联合弹性成像进行诊断,对比两者病灶声像图的特征差别以及两种方法诊断疾病的准确率等。结果共研究102例患者,病灶共145个,其中良性病灶共86个,主要以纤维腺瘤最为多发;恶性病灶59个,其中以浸润性导管癌最为多发;良、恶性病灶形态及边缘多数呈现毛刺、分叶、成角等,两者差别不具有统计学意义(P>0.05);良性病灶多数边界清晰,形态多呈分叶、毛刺征,后无回声无明显衰减,少数可见微小钙化,并且病灶内血流信号多呈0~I级,弹性评分1~3分;恶性病灶多数边界呈分叶、毛刺征,形态不规则,后无回声伴有衰减,多数病灶内可见微小钙化,病灶内血流信号多呈Ⅱ~Ⅲ级,弹性评分4~5分;超声检查病灶的良、恶性的准确率显著低于超声联合超声弹性成像,两者差别显著,具有统计学意义(P<0.05)。结论超声联合弹性成像在鉴别非哺乳期乳腺癌和乳腺炎病变中有着较高的诊断率,能够显示病灶的声像图特征表现,可以为临床提供良好的诊断依据。 Objective To investigate the diagnostic value of ultrasound combined with elasticity imaging in the identification of non-lactating mastitis and breast cancer.Methods Selected 102 cases of patients with breast lesions in our hospital from January 2016 to March 2017,they were divided into benign group and malignant group according to the postoperative pathological results,used ultrasound combined with elastic imaging for diagnosis,compared the characteristics of the lesions and accuracy of the two methods.Results There were a total of 145 lesions in 102 patients,86 cases of benign lesions and most were fibrous adenoma,59 cases of malignant lesions and most were invasive ductal carcinoma,the edge was burrs,lobes,angle in benign and malignant lesions,but there was no statistically significant(P 〉0.05).Most of the boundaries were clear,morphological lobes,burr in benign lesions,no echo after the rear,no significant attenuation,there were small calcification in few of lesions,the blood flow signal mostly 0 ~ I level,the elastic score was between 1 ~ 3 points.Most of the borders were lobulated,burr,irregular shape in malignant lesions,no echo,there were small calcification in few of lesions,the blood flow signals were II-III grade,elastic score was 4 ~ 5 points.The accuracy of ultrasonic examination of benign and malignant lesions was significantly lower than that of ultrasonography combined with ultrasound elastic imaging.The difference between the two groups was statistically significant(P 〈0.05).Conclusions Ultrasound combined with elastic imaging has a high diagnostic rate in the identification of non-lactating breast and mastitis lesions,can show the characteristics of the sonographic features of the lesion,and can provide a good clinical basis for diagnosis.
