不同肠道清洁度下升结肠染色对息肉检出率的影响

The effect of indigo carmine staining for the detection rate of ascending colon polyps at different intestinal cleanliness

目的探讨升结肠染色技术在不同肠道清洁度下对升结肠息肉检出率的影响。方法本院近半年行结肠镜检查的2356例患者,肠道清洁度评分0~1分者为A组,共1558例,2分者为B组,共798例。A组中靛胭脂染色观察升结肠者共760例(A1组),常规观察者共798例(A2组);B组靛胭脂染色观察升结肠者共412例(B1组),常规观察升结肠者386例(B2组)。对比A1组和A2组、B1组和B2组之间升结肠息肉的检出率。结果 A1组检出升结肠息肉患者58例,检出率为7.63%,A2组检出升结肠息肉34例,检出率为4.26%;B1组检出升结肠息肉28例,检出率为6.80%,B2组检出升结肠息肉患者15例,检出率为3.89%。A1组较A2组检出率较高,差异有统计学意义(P<0.05);B1组较B2组检出率较高,差异无统计学意义(P>0.05)结论升结肠染色技术,能显著提高肠道清洁度0~1分患者的升结肠息肉检出率,不能显著提高肠道清洁度评分2分患者的升结肠息肉检出率。

Objective The aim of this study was to evaluate the influence of staining technology with indigo carmine on the detection rate of polyps in the ascending colon at different intestinal cleanliness.Methods A study was done on 2356 patients in the past six months.These 2356 cases were divided into two groups according to intestinal cleanliness score.Ottawa intestinal cleanliness score of Group A was 0 ~ 1 points,including a total of 1558 cases.Intestinal cleanliness score of 798 patients in group B was 2 points.760 patients in group A received indigo rouge staining of ascending colon polyps were divided into experimental group（Group A1）,the other 798 cases were divided into the control group（Group A2）.Similarly,412 patients of group B received indigo rouge staining were included in group B1,and the other 386 patients were contained in group B2.We compared the detection rate of ascending colon polyps respectively between group A1 and A2,group B1 and B2.Results 58 patients of group A1 were found to suffered ascending colon according to indigo rouge staining,the incidence rate was 7.63%.And the other three groups were 4.26%（group A2,34 cases）,6.80%（group B1,28 cases）,3.89%（group B2,15 cases）,respectively.The detection rate of Group A1 was significantly higher than that in group A2（P〈0.05）.Even though the rate of group B1 was higher than group B2,however,there was no significant（P〈0.05）.Conclusions Staining technology with indigo carmine can significantly increase detection rate ascending colon polyp in the case of intestinal cleanliness score 0 ~ 1 points.However,it couldn＇t increase the rate in patients with 2 points of intestinal cleanliness score.
