应用PASS系统监测厦门市海沧医院临床用药合理性的效果探讨

Discussion on Effects of PASS on Monitoring the Rationality of Clinical Medication in Xiamen Haicang Hospital

目的:基于合理用药监测系统(prescription automatic screening system,PASS)的监测结果,探讨厦门市海沧医院(以下简称"我院")临床用药合理性,以提高临床合理用药水平。方法:通过PASS,对我院2017年1—6月的问题医嘱进行统计分析。结果:共监测问题医嘱20 776条,其中,黑灯提示284条(占1.37%),红灯提示16 752条(占80.63%),橙灯提示3 740条(占18.00%);最常见的为儿童用药问题(6 094条,占29.33%),其次为剂量范围(5 045条,占24.28%)。结论:PASS能有效监测临床不合理用药情况。但临床仍存在较多不合理用药问题,且PASS也存在一些缺陷,需要加强药师审核及介入。

OBJECTIVE： On the basis of the monitoring results of prescription automatic screening system（ PASS）,to probe into the rationality of clinical medication in Xiamen Haicang Hospital（ hereinafter referred to as＂our hospital＂）,so as to improve clinical rational medication. METHODS： By means of PASS,irrational medical orders in our hospital from Jan. to Jun. 2017 were collected and analyzed. RESULTS： A total of 20 776 irrational medical orders were monitoring,of which 284 medical orders with black light hint（ 1. 37%）,16 752 with red light hint（ 80. 63%）,3 740 with orange light hint（ 18. 00%）; mostly were caused by irrational medication on children（ 6 094 medical orders, 29. 33%）, the secondly was inappropriate dosage range（ 5 045, 24. 28%）.CONCLUSIONS： PASS can effectively monitoring irrational clinical medication. While there still exists irrational medication and defects in PASS,so that it is necessary to strengthen the audit and intervention by pharmacists.
