对四川甘孜11家医院住院患者不合理用药情况的调查分析

Investigation and analysis on the unreasonable use of drugs in 11 hospitals in ganzi, sichuan

目的:调查分析四川甘孜藏族自治州11家综合医院住院患者不合理用药的情况,以期提高该地区综合医院住院患者用药的安全性和有效性。方法:随机抽取四川甘孜藏族自治州11所综合医院2016年1月至2017年6月期间的110份出院病历(每所医院抽取10份病历),以其中包含的4735条医嘱(含临时医嘱)为调查对象。对这4735条医嘱中不合理用药的情况进行分析。结果及结论:在这4735条医嘱中,有968条医嘱存在不合理用药的情况,占医嘱总数的20.44%。这11所综合医院住院患者存在的不合理用药问题主要包括重复用药、无适应证用药、用药的适应证不适宜、遴选药品不适宜、药品剂型或给药途径不适宜及联合用药不适宜。加强对民族地区综合医院住院患者用药物情况的管理,加大对该地区综合医院临床医生和药师的培训力度是提高该地区综合医院用药的合理性,保障住院患者用药安全的重要措施。

objective： to investigation and analysis of sichuan ganzi Tibetan autonomous prefecture of 11 patients hospitalized in general hospitals is not rational drug use, in order to improve the region comprehensive hospital patients medication safety and effectiveness. Methods： a random sample of sichuan ganzi Tibetan autonomous prefecture 11 general hospital between January 2016 and June 2016, during the period of 110 discharged medical records（in every hospital medical records for 10）, in which contains 4735 orders（including temporary orders） as the investigation object. The situation of unreasonable use of drugs in the 4,735 medical orders was analyzed. Results and conclusion： among the 4,735 medical orders, 968 medical orders had unreasonable use of drugs, accounting for 20.44% of the total. The 11 patients hospitalized in general hospitals with the irrational drug use problems mainly include repeated drug use, there is no indication of medication, medication indications are not appropriate, selected drugs not suitable dosage, dosage form, or is not appropriate and combination are not appropriate. To strengthen national regions with drugs in patients with general hospital in hospital management, increase the district general hospital clinical doctors and pharmacists training is to enhance the rationality of general hospital drug use in the region, of the important measures to protect the safety of medication in hospitalized patients.
