多囊卵巢综合征患者的胰岛素抵抗临床特征及相关因素分析

Analysis of Clinical Features and Related Factors of Insulin Resistance of Patients with Polycystic Ovary Syndrome

目的对多囊卵巢综合征(PCOS)患者胰岛素临床特征及相关因素进行探讨。方法方便选取该院2015年2月—2017年2月收治的PCOS患者86例进行研究,分为观察组与对照组,各43例,对照组为非胰岛素抵抗患者,观察组为胰岛素抵抗患者。对两组患者相关指标进行对比观察,分析相关因素。结果在体质指数、TG、HDL水平方面,观察组分别为(26.83±2.96)kg/m^2、(2.26±0.42)mmol/L、(1.61±0.37)mmol/L,均明显高于对照组,差异有统计学意义(P<0.05);在TC、LDL、P、T、E2、PRL、LH、FSH水平方面,两组差异无统计学意义(P>0.05);在FPG、FINS、GAUC、HOMA-IR水平方面,观察组分别为(5.23±0.65)mmol/L、(18.77±10.93)m IU/L、(20.53±3.48)、(4.30±2.64),均明显高于对照组,差异有统计学意义(P<0.05);对于PCOS胰岛素抵抗患者而言,HOMA-IR与BMI、GAUC呈正相关。结论 PCOS胰岛素抵抗患者HOMA-IR与BMI、GAUC呈正相关,降低患者葡萄糖耐量及肥胖,可使患者发病风险增加。

Objective To study the clinical features and related factors of insulin resistance of patients with polycystic ovary syndrome. Methods 86 cases of PCOS patients admitted and treated in our hospital from February 2015 to February 2017 were convenient selected and divided into two groups with 43 cases in each, the control group were the non-insulin resistance patients, while the observation group were the insulin resistance patients, and the related indicators were compared,and the related factors were analyzed. Results The body mass index, TG, and HDL in the observation group were respectively（26.83±2.96）kg/m^2,（2.26±0.42）mmol/L,（1.61±0.37）mmol/L, which were obviously higher than those in the control group,and the differences were statistically significant（P〈0.05）, and there were no obvious differences in the TC,LDL,P,T,E2,PRL,LH,FSH levels between the two groups（P〈0.05）, and the FPG,FINS,GAUC,HOMA-IR levels in the observation group were respectively（5.23±0.65）mmol/L,（18.77±10.93）m IU/L,（20.53±3.48）,（4.30±2.64）, which were obviously higher than those in the control group, and the differences were statistically significant（P〈0.05）, and the HOMA-IR of patients with PCOS patients with insulin resistance was positively correlated with the BMI and GAUC. Conclusion The HOMA-IR of patients with PCOS patients with insulin resistance is positively correlated with the BMI and GAUC, which can decrease the glucose tolerance and obesity and increase the onset risk of patients.
