呼吸功能锻炼及有效咳嗽对COPD患者排痰效果的影响

Effect of Respiratory Function Training and Effective Cough on the Sputum Excretion of COPD Patients

目的比较呼吸功能锻炼及有效咳嗽前后COPD患者排痰的效果及动脉血气指标的变化。方法方便选择2015年9—10月该院就诊的COPD患者120例,将只接受常规治疗的患者做为对照组,在常规治疗的基础上实施有效咳嗽及呼吸功能锻炼的患者做为实验组,观察两组的排痰效果及动脉血气分析的变化。结果两组排痰量以及动脉血气(pH、PaCO_2、PaO_2、SaO_2)结果比较差异有统计学意义(P<0.05)。实验组第9天排痰量(56.00±12.63)mL较对照组(46.75±10.30)mL高,差异有统计学意义(t=4.396,P=0.000);实验组第5天、第9天PO_2(67.65±8.20)、(73.15±7.47)mmHg均较该组入院当天高,差异有统计学意义(t=4.739、8.304,P=0.000)。结论呼吸功能锻炼及有效咳嗽技巧的掌握有利于COPD患者顺利排出痰液,维持呼吸道通畅,防止肺不张等并发症,对于改善PO_2和SaO_2起到非常重要的作用。

Objective To compare the changes of sputum excretion effect and arterial blood gas indicators of COPD patients before and after the respiratory function training and effective cough. Methods 120 cases of COPD patients diagnosed in our hospital from September 2015 to October 2015 were convenient selected and divided into two groups, the control group only used the routine therapy, while the experimental group used the respiratory function training and effective cough on the basis of the control group, and the sputum excretion effect and arterial blood gas indicators of the two groups were observed.Results The differences in the sputum excretion volume and pH,PaCO2, PaO2, SaO2 between the two groups were statistically significant（P〈0.05）, and the sputum excretion volume on the ninth day in the experimental group was higher than that in the control group（56.00±12.63）mL vs（46.75±10.30）mL], and the difference was statistically significant（t=4.396,P=0.000）;and the PO2 on the 5 day and 9 day in the experimental group was respectively（67.65 ±8.20）mmHg,（73.15 ±7.47）mmHg,which were higher than those on the day, and the differences were statistically significant（t=4.739,8.304,P=0.000）. Conclusion The respiration function training and mastery of effective cough skills is conducive to the successful sputum excretion of COPD patients, maintain the smoothness of respiration tract, and prevent the complications such as atelectasis, which is of very important significance to improving the PO2 and SaO2.
