溃疡性结肠炎患者血清自身抗体检测的价值分析

Analysis of Value of Serum Autoantibody Test of Patients with Ulcerative Colitis

目的探讨溃疡性结肠炎(UC)患者血清自身抗体检测的价值。方法方便选取2015年2月—2016年6月收治的68例溃疡性结肠炎患者,设定为观察组,并选取同时期肠易激综合征患者68例,设定为对照组,观察两组血清抗中性粒细胞胞浆抗体(p-ANCA)、抗小肠杯状细胞抗体(GAB)、抗酿酒酵母菌抗体(ASCA)及抗胰腺腺泡抗体(PAB)对UC的检测情况。结果观察组血清p-ANCA、GAB检测阳性率(58.82%、32.35%)均高于对照组(1.47%、8.82%)(P<0.05);观察组p-ANCA、GAB、ASCA、PAB联合检测阳性率(76.47%)明显高于对照组(14.71%)(P<0.05);p-ANCA与其他3种分别联合检测阳性率均提高,且p-ANCA/GAB联合检测对比单项检测的阳性率提高较为显著,与四项联合检测结果对比差异无统计学意义(P>0.05);两组联合检测结果对比差异有统计学意义(P<0.05)。结论 p-ANCA、GAB对UC的临床诊断具有明显作用,且其联合检测价值更高,PAB、ASCA无显著作用。

Objective To study the value of serum autoantibody test of patients with ulcerative colitis. Methods 68 cases of patients with ulcerative colitis admitted and treated in our hospital from February 2015 to June 2016 were convenient selected as the observation group, 68 cases of patients with irritable bowel syndrome admitted and treated at the same period were selected as the control group, and the test of UC by the serum p-ANCA, GAB, ASCA and PAB of the two groups was observed. Results The positive rates of serum p-ANCA and GAB in the observation group were higher than those in the control group,（58.82%,32.35% vs 1.47%,8.82%）（P〈0.05）, and the positive rate of p-ANCA,GAB,ASCA,PAB was obviously higher than that in the control group,（76.47% vs 14.71%）（P〈0.05）, and the positive rate of p-ANCA and the other three types was improved, and the positive rate of p-ANCA/GAB was improved more obviously compared with that of single test,and the difference between the p-ANCA/GAB test and four combined test was not obvious（P〉0.05）, and the difference in the combined test results between the two groups was obvious（P〈0.05）. Conclusion P-ANCA and GAB have an obvious effect on the clinical diagnosis of UC, and the combined test value is higher, and the PAB and ASCA have no obvious effect.
