应用全髋关节置换术和半髋关节置换术治疗老年股骨颈骨折的疗效对比分析

Analysis of Comparison of Curative Effect of Hip Joint Replacement and Hemiarthroplasty in Treatment of Senile Transcervical Fracture

目的研究全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的临床效果。方法方便选取该院2014年12月—2017年8月期间所收治的74例老年股骨颈骨折患者作为该次研究病例,按照随机数字表法将其分为研究组和对照组,每组37例,对照组应用半髋关节置换术治疗,研究组应用全髋关节置换术治疗,对比分析两组患者的临床效果,比较两组患者的手术时间与手术中出血量,观察两组患者手术后髋关节功能恢复时间与手术后疼痛率。结果研究组临床优良率为97.29%,对照组优良率为67.57%,两组差异有统计学意义(χ2=9.765 4,P=0.001 7),且研究组手术时间与手术中出血量均明显多于对照组,差异有统计学意义(t=9.635 3;13.305 9;0.688 5,P<0.05),两组在术后疼痛率与髋关节功能恢复时间方面差异无统计学意义。结论全髋关节置换术与半髋关节置换术应用在老年股骨颈骨折中的效果均有明显效果,临床中应根据患者的身体状态与并发症选择合适的手术方式。

Objective To study the clinical effect of hip joint replacement and hemiarthroplasty in treatment of senile transcervical fracture. Methods 74 cases of senile patients with transcervical fracture admitted and treated in our hospital from December 2014 to August 2017 were selected and randomly divided into two groups with 37 cases in each, the control group and the research group were respectively treated with hemiarthroplasty and total hip replacement,and the clinical effect was compared between the two groups and the operation time, intraoperative bleeding amount, were compared between the two groups, and the hip joint function recovery time and pains rate after surgery of the two groups were observed.Results The clinical excellent and good rate in the research group and in the control group was respectively 97.29% and67.57%, and the difference between the two groups was obvious（χ^2=9.765 4,P=0.001 7）, and the operation time and intraoperative bleeding amount in the research group were obviously more than those in the control group, and the differences were obvious, with statistical significance（t=9.635 3,13.305 9,P〈0.05）, and the differences in the postoperative pains rate and hip joint function recovery time between the two groups were not statistically significant. Conclusion The application effect of hip joint replacement and hemiarthroplasty in treatment of senile transcervical fracture is obvious, and we should select the proper operation way according to the physical state and complications of patients.
