右美托咪定在手术室硬膜外镇痛麻醉中的应用效果

Application Effect of Dexmedetomidin in the Anaesthesia of Epidural Analgesia in the Operating Room

目的对手术室硬膜外镇痛麻醉的过程中行右美托咪定干预的应用效果进行研究及判定。方法方便选取该院收治的60例手术室硬膜外麻醉患者作为该次的研究对象,收治时间在2015年2月—2017年1月期间,依据麻醉方案分为对照组、研究组,2组手术室硬膜外麻醉患者各30例,对照组行罗哌卡因(c:0.5%)15 m L联合Na Cl溶液(c:0.9%)1 m L辅助硬膜外麻醉,研究组行硬膜外注射50μg右美托咪定及Nacl溶液稀释液及15 m L罗哌卡因(c:0.5),并将2组患者经相应麻醉方案干预的术后4、12、24及48 h的VAS疼痛评分、镇痛总有效率及不良反应发生情况进行对比。结果研究组手术室硬膜外麻醉患者经麻醉干预的术后12 h的VAS评分为(2.12±0.36)分、术后24 h的VAS评分为(1.98±0.25)分,均低于对照组(t12 h=20.62,t24 h=22.40,P<0.05),镇痛总有效率为96.67%,高于对照组(χ2=5.19,P=0.02),且不良反应发生率为6.67%,与对照组对比差异无统计学意义(χ2=0.22,P=0.64)。结论对手术室硬膜外镇痛麻醉的过程中行右美托咪定干预的应用效果显著。

Objective To research and determine the application effect of dexmedetomidin in the anaesthesia of epidural analgesia in the operating room. Methods 60 cases of patients with anaesthesia of epidural analgesia in the operating room from February 2015 to January 2017 were convenient selected and divided into groups according to the anesthesia plan with30 cases in each, the control group used the ropivacaine（c：0.5%）15 mL combined with Na Cl solution（c：0.9%）1 mL, while the research group used the epidural injection of 50 μg dexmedetomidin and Nacl solution and 15 mL ropivacaine（c：0.5）,and the VAS scores at 4 h, 12 h, 24 h and 48 h after the corresponding anesthesia plans, total analgesia effective rate and occurrence of adverse reactions were compared between the two groups. Results The VAS score at 12 h after surgery after the anesthesia intervention in the operating room and VAS score at 24 h after surgery in the research group were respectively（2.12±0.36）marks and（1.98±0.25）marks, which were lower than those in the control group（t12 h=20.62,t24 h=22.40,P〈0.05）,and the total analgesia effective rate was 96.67%, which was higher than that in the control group（χ^2=5.19,P=0.02）, and the incidence rate of adverse reactions was 6.67%, and the differences between the two groups were not obvious（χ^2=0.22,P=0.64）.Conclusion The application effect of dexmedetomidin in the anaesthesia of epidural analgesia in the operating room is obvious.
