老年慢性心力衰竭患者衰弱现况及影响因素分析

Frailty and its influencing factors of elderly patients with chronic heart failure

目的了解老年慢性心力衰竭(CHF)患者的衰弱现况及影响因素。方法采用自行设计的一般资料调查表、老年人衰弱评估量表对241例老年慢性心力衰竭患者进行调查。结果老年慢性心力衰竭患者的衰弱评分为21.17±3.92,36.5%的患者发生衰弱。不同年龄、文化程度、职业、医疗付费方式、心功能分级的患者衰弱得分比较,差异有统计学意义(均P<0.01),其中年龄、文化程度、心功能分级是影响患者衰弱的主要因素(调整R2=0.552)。结论老年慢性心力衰竭患者衰弱发生率较高,须建立和完善适用于老年CHF患者的衰弱评估及干预机制,以早发现早干预。

Objective To investigate the status of frailty among elderly patients with chronic heart failure（CHF）,and to explore the effects of different demographic factors on frailty.Methods A self-designed general information questionnaire and the Elderly Frailty Assessment Scale were administered to 241 elderly CHF patients from 4 hospitals in Hengyang.Results The overall frailty score among elderly CHF patients averaged 21.17±3.92,and 36.5% of the elderly patients had frailty.There were significant differences in frailty scores among patients at different age groups,education levels,professions,or with different ways of medication payments and cardiac function classification（P〈0.01 for all）.Multiple linear regression analysis revealed that age,education levels and cardiac function classification were influencing factors of frailty for elderly CHF patients（adjusted R^2=0.552）.Conclusion The prevalence of frailty in elderly CHF patients is high,and it is imperative to establish and perfect frailty assessment and intervention mechanism for early detection and management.
