消化道黏膜下层病变的超声内镜影像学特点及其临床价值

Imaging features of endoscopic ultrasonography and its clinical value for submucosal lesions of digestive tract

目的探讨消化道黏膜下层病变的超声内镜影像学特点及其临床价值。方法回顾性分析81例消化道黏膜下层病变患者超声内镜声像学特征,并以病理诊断为金标准,评价其诊断效能。结果超声内镜下异位胰腺以混合回声为主,脂肪瘤表现为均匀高回声,间质瘤表现为均匀或不均匀低回声,平滑肌瘤表现为均匀低回声。超声内镜对黏膜下层病变性质的检出率为63.0%,其中诊断异位胰腺、脂肪瘤、间质瘤和平滑肌瘤的敏感性分别为74.1%、84.6%、55.6%和50.0%,特异性分别为81.5%、80.0%、94.4%和97.4%。结论对于消化道黏膜下层病变,超声内镜具有良好的临床价值,是必要的诊断方法。

Objective To explore the imaging features of endoscopic ultrasonography（ EUS） and its clinical value for submucosal lesions of digestive tract. Methods The imaging features of EUS for submucosal lesions of digestive tract were retrospectively analyzed among the 81 patients. The pathological findings were used as gold standard to assess the diagnostic efficiency of EUS. Results EUS revealed that mixed echo was common in ectopic pancreas,uniform hyperecho in lipoma,uniform or nonuniform hypoecho in stromal tumors,and uniform hypoecho in leiomyoma. The detection rate of EUS for diagnosing the features of submucosal lesions of digestive tract was63. 0%,the sensitivities for diagnosing ectopic pancreas,lipoma,stromal tumors and leiomyoma were 74. 1%,84. 6%,55. 6% and 50. 0%respectively,and the specificities were 81. 5%,80. 0%,94. 4% and 97. 4% respectively. Conclusion For submucosal lesions of digestive tract,EUS is an essential diagnostic method with a favorable clinical value.
