复方四黄液湿敷对胫骨平台骨折术后疼痛肿胀及功能康复的影响

Efficacy of the Fufang Sihuang liquid on pain, swelling and function rehabilitation after tibial plateau fracture

目的:观察复方四黄液湿敷对胫骨平台骨折术后膝关节疼痛肿胀及功能康复的影响。方法:选取2015年1月-2017年2月中山市中医院显微创伤科诊断为胫骨平台骨折的患者80例,随机分为对照组与观察组,各40例,对照组及观察组术后均给常规治疗及护理,观察组在此基础上予复方四黄液湿敷;分别观察并比较两组术后第1、7、14天膝关节的疼痛程度及术后第7、14、30天的肿胀程度,并对比两组患者术后3个月膝关节功能康复情况。结果:所有病例均得到有效随访,观察组术后第7、14天的疼痛评分(VAS)低于对照组,差异有统计学意义(P<0.05),而术后第1天两组VAS评分无明显差异(P>0.05);观察组术后第7、14天及30天肿胀程度均低于对照组,差异有统计学意义(P<0.05);术后3个月,膝关节功能等级分布显示观察组优、良、中、差分别为24、12、3、1例,对照组分别为15、14、7、4例,两组患者临床疗效分布比较,观察组优于对照组,差异有统计学意义(P<0.05)。结论:胫骨平台骨折术后早期应用复方四黄液湿敷可明显改善患肢肿胀及疼痛症状,从长远效果来看,对膝关节功能康复具有促进作用,值得临床运用和推广。

Objective： To observe efficacy of the Fufang Sihuang liquid（复方四黄液） on pain, swelling and function rehabilitation after tibial plateau fracture. Methods： 80 cases were divided into two groups, 40 cases in each group. Two groups were given conventional nursing and treatment. The observation group took the Fufang Sihuang liquid more. Results： All cases were followed up effectively. The pain scores（VAS） on the 7 th and 14 th days after the operation in the observation group were lower than those in the control group（P〈0.05）. On the first day, the VAS scores were similar between two groups（P〈0.05）. The degree of swelling in the observation group on the 7 th, 14 th, and 30 th day was lower than that in the control group（P〈0.05）. In 3 months, 24 cases were excellent the knee function, 12 cases were good, 3 cases were moderate and 1 case was poor in the observation group. In the control group, they were 15 cases, 14 cases, 7 cases and 4 cases respectively. The effect in the observation group was better（P〈0.05）. Conclusion： The Fufang Sihuang liquid in early can significantly improve the symptoms of limb swelling and pain, promote the recovery of knee joint function and is worthy of clinical application and promotion.
