后路经伤椎椎弓根钉短节段固定治疗胸腰椎骨折的疗效及对术后疼痛的影响

Efficacy of posterior short-segment pedicle screw fixation of injured vertebra for thoracolumbar fractures and its effect on postoperative pain

目的探究后路经伤椎椎弓根钉短节段固定治疗胸腰椎骨折的疗效及对术后疼痛的影响。方法选取我院2015年7月至2017年7月收治的胸腰椎骨折患者92例,按随机数字表法分为2组,各46例。对照组采用跨伤椎短节段椎弓根钉固定治疗,观察组采用后路经伤椎椎弓根钉短节段固定治疗。对比2组术后3 d出血量、临床疗效及手术前后疼痛情况变化。结果观察组术后3 d出血量少于对照组,差异有统计学意义(P<0.05);观察组治疗后椎体压缩程度轻于对照组,伤椎Cobb角小于对照组,差异有统计学意义(P<0.05);观察组术后疼痛程度轻于对照组,差异有统计学意义(P<0.05)。结论后路经伤椎椎弓根钉短节段固定治疗胸腰椎骨折可有效减少术后出血,并提高临床疗效,缓解患者术后疼痛。

Objective To explore the efficacy of posterior short-segment pedicle screw fixation of injured vertebra for thoracolumbar fractures and its effect on postoperative pain. Methods A total of 92 patients with thoracolumbar fractureswere randomly divided into 2 groups with 46 cases in each group according to the random number table, who were admitted into our hospital from July 2015 to July 2017. The control group was treated with short-segment pedicle screw fixation across the injured vertebrae, and the observation group was treated with posterior short-segment pedicle screw fixation of the injured vertebrae. The blood loss, clinical efficacy and preoperative and postoperative pain changes were compared between the two groups 3days after the operation. Results The blood loss 3days after the operation in the observation group was lower than that in the control group, and the difference between the two groups was statistically significant（ P 〈 0.05 ）. The degree of vertebral compression in the observation group was less severe than that in the control group, the Cobb＇ s angle of the injured vertebra in the observation group was smaller than that in the control group and the difference between the two groups was statistically significant （P 〈0.05）. The postoperative pain in the observation group was less severe than that in the control group,and the difference between the two groups was statistically significant（P 〈 0.05）. Conclusion The posterior short-segment pedicle screw fixation of injured vertebra for thoracolumbar fractures can effectively reduce postoperative hemorrhage,improve clinical efficacy and relieve postoperative pain.
