金福安汤联合TP方案治疗对Ⅲ、Ⅳ期非小细胞肺癌脾虚痰湿型患者中医症候积分、生存质量及血清CEA、CY211水平的影响

Effect of Jinfuan Decoction Combined with TP Regimen on Traditional Chinese Medicine Syndrome Scores,Quality of Life,Serum CEA and CY211 Levels in Patients with Stage Ⅲ and Ⅳ Non-small Cell Lung Cancer of Spleen Deficiency and Phlegm Dampness Type

目的:观察金福安汤联合紫杉醇+顺铂(TP)方案治疗对Ⅲ、Ⅳ期非小细胞肺癌(NSCLC)脾虚痰湿型患者中医症候积分、生存质量及血清癌胚抗原(CEA)、骨胶素(CY211)水平的影响。方法:按随机数字表法将120例NSCLC患者分为对照组与观察组,每组60例。对照组予以TP方案治疗,观察组在对照组基础上联合金福安汤剂治疗。观察两组患者临床疗效、中医症状及安全性,评价治疗前后患者生存质量变化,测定治疗前后患者血清CEA、CY211水平。结果:观察组总有效率及稳定率显著高于对照组(P<0.05);治疗后两组患者中医症状评分显著下降(P<0.05),生活质量积分显著升高(P<0.05)治疗后观察组中医症状及生活质量显著优于对照组(P<0.05);治疗后两组患者血清CEA、CY211水平显著降低(P<0.05),治疗后观察组与对照组CEA、CY211水平差异显著(P<0.05)。此外,观察组恶心、呕吐、腹泻、心电图异常及中性粒细胞减少和血小板下降情况发生率低于对照组,差异显著(P<0.05)。结论:金福安汤联合化疗治疗可有效改善NSCLC患者中医症状,提高治疗效果及患者的生存质量,并能减少化疗药物引起的不良反应。

Objective： To study the effect of Jinfuan Decoction combined with paclitaxel plus cisplatin（ TP） regimen on TCM syndrome scores,quality of life,serum carcinoembryonic antigen（ CEA） and CY211 levels in patients with stage III and IV non-small cell lung cancer（ NSCLC） of spleen deficiency and phlegm dampness type. Methods： 120 patients with NSCLC were divided into controlled group and observation group by the random number table method,with 60 cases in each group. The controlled group was treated with TP regimen while the observation group was treated with Jinfuan Decoction,based on the treatment of the controlled group. The clinical curative effect,TCM symptoms and safety in the two groups were observed. Changes of quality of life before and after treatment were evaluated. The levels of serum CEA and CY211 were measured before and after treatment.Results： The total effective rate and stability rate in the observation group were significantly higher than that in the controlled group（ P〈0. 05）. After treatment,the TCM symptom scores were decreased significantly（ P〈0. 05）,while the quality of life scores were increased significantly（ P〈0. 05）. After treatment,symptoms and quality of life in the observation group were significantly better than those in the controlled group（ P〈0. 05）. After treatment,the levels of CEA and CY211 in the two groups were significantly decreased,with significant differences within and between groups（ P〈0. 05）. The incidence rates of nausea,vomiting,diarrhea,electrocardiogram abnormalities,neutropenia and thrombocytopenia in the observation group were lower than those in the controlled group（ P〈0. 05）. Conclusion： Jinfuan Decoction combined with chemotherapy can effectively improve the TCM symptoms of patients with NSCLC,improve the curative effect and the quality of life of patients,and reduce adverse reactions caused by chemotherapy drugs.
