中医情志护理对肾虚血瘀型老年股骨转子间骨折患者术后骨代谢生化指标影响分析

Effect of Traditional Chinese Medicine Care on Biochemical Indexes of Bone Metabolism in Elderly Patients with Intertrochanteric Fracture of Femur with Kidney Deficiency and Blood Stasis

目的:观察中医情志护理对肾虚血瘀型老年股骨转子间骨折患者术后骨代谢生化指标影响。方法:选取2015年5月至2017年5月间在本院接受PFNA手术治疗的肾虚血瘀型老年股骨转子间骨折患者133例,依据随机数字表法将其分成两组,对照组(67例)采用常规护理,观察组(66例)在对照组基础上采用中医情志护理;观察手术前后患者髋关节Harris评分、骨代谢生化指标及VAS评分改变状况。结果:观察组患者优良率为87.88%,高于对照组的65.67%,差异有统计学意义(P<0.05);对照组血清ALP术后2周含量较治疗前显著降低,观察组术后1、2周VAS评分较术前、对照组显著降低,差异均有统计学意义(P<0.05);观察组术后12、24周腰椎BMD、髋关节Harris较手术前和对照组显著升高,差异有统计学意义(P<0.05)。结论:中医情志护理可显著改善老年股骨转子间骨折患骨代谢,减轻术后疼痛状况,加速髋关节康复,效果值得肯定。

Objective： To analyze the effect of traditional Chinese medicine nursing on postoperative biochemical markers of bone metabolism in elderly patients with intertrochanteric fractures of kidney deficiency and blood stasis. Methods： A total of133 elderly patients with intertrochanteric fractures of the kidney with blood stasis syndrome treated by PFNA in our hospital from May 2015 to May 2017 were divided into two groups,according to the random number table method. The controlled group（ 67 patients） was treated with routine nursing,and the observation group（ 66 cases） were treated with traditional Chinese medicine emotional nursing on the basis of the controlled group. Harris score,biochemical indexes of bone metabolism and changes of VAS scores were observed before and after operation. Results： Patients with good rate of 87. 88%,higher than 65. 67% in the controlled group,the difference was statistically significant（ P〈0. 05）; The controlled group of serum ALP after two weeks was significantly lower than before treatment,the observation group one or two weeks after operation,VAS score were significantly reduced,the controlled group,the differences were statistically significant（ P〈0. 05）; The observation group after twelve and twenty-four weeks of lumbar BMD,hip Harris before surgery and the controlled group were significantly increased,the difference was statistically significant（ P〈0. 05）. Conclusion： TCM emotional nursing can significantly improve bone metabolism,reduce postoperative pain and accelerate hip rehabilitation in elderly patients with intertrochanteric fractures.
