大承气汤化裁保留灌肠对脓毒症患者凝血功能的影响

The effect of retention enema with Dachengqi Decoction on blood coagulation in patients with sepsis

目的 探讨大承气汤化裁保留灌肠对脓毒症患者凝血功能的影响。方法 本研究采取随机对照原则，选取符合纳入标准的60例脓毒症患者，试验组及对照组各30例。试验组应用大承气汤化裁保留灌肠结合西医常规治疗，对照组应用温水保留灌肠及西医常规治疗。患者在纳入时、治疗后第3天、第7天抽取外周静脉血进行凝血四项（APTT、PT、TT和FIB）、D-二聚体指标的检测。结果 试验组大承气汤化裁保留灌肠治疗后第3天、第7天，凝血四项、D-二聚体的变化幅度与对照组比较，差异有统计学意义（P〈0.05）。且大承气汤化裁保留灌肠治疗后第7天与第3天检测指标水平比较改善较为明显。结论 大承气汤化裁保留灌肠可以有效地改善脓毒症患者凝血四项、D-二聚体水平。

Objective To investigate the effect of retention enema with Dachengqi Decoction on blood coagulation in patients with sepsis. Methods In this study, a randomized controlled trial was conducted in 60 patients who met the inclusion criteria. The experimental group and the control group each had 30 cases. The experimental group was treated with Dachengqi Decoction retention enema and routine western medicine, while the control group was treated with warm water retention enema and routine western medicine treatment. The peripheral blood and vein blood were collected on the day of enrollment, on the third day and seventh day after treatment, and four items of blood coagulation （ APTT, PT, TT, FIB） and D - dimer were detected. Results The experimental group of Dachengqi Decoction retention enema on the third day, seventh day of treatment, four items of blood coagulation, two D - dimer changes compared with the control group, the difference was statistically significant （ P 〈 0.05 ）. And the Dachengqi Decoction retention enema treatment showed further improved test index levels on the seventh day, compared with those on the third day. Conclusion Dachengqi Decoction and modified retention enema can effectively improve the coagulation four and D - dimer levels in patients with sepsis.
