摘要
目的检测细胞粘附分子CD146在正常子宫内膜、子宫内膜癌中的表达及临床意义。方法收集郑州大学第五附属医院2005年10月—2016年10月实验标本102例,经病理证实后分为子宫内膜癌组63例、正常子宫内膜组39例,应用免疫组化法检测CD146在子宫内膜组织上的表达及与其病理参数的关系,包括FIGO分期、组织学分级、肌层浸润深度。结果 CD146在子宫内膜癌、正常子宫内膜中的阳性表达率为63.49%、5.13%,差异有统计学意义(χ~2=36.328,P<0.05)。在子宫内膜癌中,高、中、低分化分组中,CD146的阳性率分别为30.43%、60.87%、94.11%,差异有统计学意义(χ~2=16.362,P<0.05),组间两两比较,均差异有统计学意义(χ~2=4.293、13.720、4.126,P<0.05);CD146阳性表达率在子宫内膜癌Ⅲ~Ⅳ期组高于Ⅰ~Ⅱ期组,肌层浸润深度≥1/2组高于<1/2组,均差异有统计学意义(χ~2=7.736、10.650,P<0.05)。结论 CD146可能在子宫内膜癌的发生、发展过程中起着关键作用,CD146检测有助于子宫内膜癌的预后判断。
Objective This paper tries to evaluate the expression of cell adhesion molecules in endometrial tissue of normal endometrium, endometrial atypical hyperplasia and endometrial cacinoma. Methods A total of 63 cases of endometrial cancer specimens were collected from October 2005 to October 2016 in the Fifth Affiliated Hospital of Zhengzhou University. The expression of CD146 was detected by immunohistochemistry and the relationship between CD146 and pathological features of the endometrial carcinoma were observed and analyzed. Results The positive rates of CD146 expression in endometrial carcinoma and normal endometrium were 63.49% and 5.13%, respectively, with significant difference(cm^2=36.328, P〈0.05).In endometrial cancer group, the positive rates of CD146 in high, moderate and poorly differentiated groups were 30.43%, 60.87% and 94.11%, the difference was statistically significant(cm^2=16.362,P〈0.05); comparison between groups,the differ wore statisticaly significant(cm^2=4.293、13.720、4.126,P〈0.05)the positive rate of CD146 expression in stage Ⅲ to Ⅳ group,higher than Ⅰ to Ⅱ group, the depth of myometrial invasion≥1/2 group,Higber than 1/2 group,the differences were statistically(cm^2=7.736,10.650, P〈0.05).Conclusion CD146 may play a key role in the tumorigenesis and development of endometrial carcinoma. The detection of CD146 contributes to the prognosis of endometrial cancer.
