穴位注射对缓解中风偏瘫肢体痉挛的临床研究

Clinical Study on Acupoint Injection in Relieving the Apoplectic Hemiplegic Limb Spasm

目的探讨穴位注射治疗对缓解中风偏瘫肢体痉挛的临床效果。方法将该院2015年1月—2017年1月收治的中风偏瘫肢体痉挛患者60例作为研究对象,随机分组,每组30例,对照组接受常规针刺与康复训练,研究组则采取穴位注射与康复训练。对两组患者临床疗效进行评价,测量治疗前、后改良Ashworth痉挛分级、FuglMeyer运动功能评分、Barthel指数评分,实施统计学分析。结果研究组总有效率高达93.33%,显著高于对照组70.00%(χ2=7.447 6,P=0.024 1<0.05);两组患者治疗前改良Ashworth痉挛分级差异无统计学意义,治疗后研究组明显优于对照组;两组患者治疗前Fugl-Meyer运动功能评分(40.83±19.56)分vs(40.27±19.84)分、Barthel指数评分(45.76±17.66)分vs(45.39±18.11)分,差异无统计学意义(t=0.110 0、0.080 1,P=0.9127、0.9364>0.05),治疗后研究组明显高于对照组(69.22±17.65)分vs(55.41±19.05)分,(70.94±20.46)分vs(60.05±18.75)分(t=2.912 6、2.149 3,P=0.0051、0.035 8<0.05)。结论穴位注射治疗可更好地缓解中风偏瘫肢体痉挛症状,提高疗效,改善肌张力、肢体运动功能及日常活动能力,值得借鉴。

Objective To study the clinical effect of acupoint injection in relieving the apoplectic hemiplegic limb spasm. Methods 60 cases of patients with apoplectic hemiplegic limb spasm admitted and treated in our hospital from January 2015 to January 2017 were selected and randomly divided into two groups with 30 cases in each, the control group used the routine acupuncture and rehabilitation training, while the research group used the acupoint injection and rehabilitation training, and the clinical curative effect was evaluated, and the modified Ashworth spasm classification,Fugl-Meyer motor function score and Barthel index score before and after treatment were measures, for statistical analysis. Results The total effective rate in the research group was obviously higher than that in the control group,（93.33% vs 70.00%）,（χ2=7.4476,P=0.02410.05）, there was no difference in the modified Ashworth spasm classification between the two groups before treatment, after treatment, the modified Ashworth spasm classification in the research group was obviously better than that in the control group; before treatment, the Fugl-Meyer motor function score and Barthel index score between the two groups, were [（40.83 ±19.56）marks vs（40.27 ±19.84）marks,（45.76 ±17.66）marks vs（45.39±18.11）marks,the difference was no statistically significant was statistically（t=0.110 0,0.080 1,P=0.912 7,0.936 40.05）], after treatment, the indicators in the research group were obviously higher than those in the control group（69.22±17.65）marks vs（55.41±19.05）marks,（70.94±20.46）marks vs（60.05±18.75）marks,the difference significant（t=2.9126, 2.1493,P =0.0051, 0.0358 0.05）]. Conclusion The acupoint injection can better relieve the apoplectic hemiplegic limb spasm, improve the curative effect, and muscular tension, limbs motor function and daily living abilities,and it is worth reference.
