缺血性卒中后认知障碍与颗粒蛋白前体(PGRN)及分泌型卷曲相关蛋白5(SFRP5)的相关性研究

摘要 目的探讨缺血性脑卒中后认知障碍患者血清中PGRN及SFRP5表达水平及与认知障碍的相关性。方法选择2016年7月—2017年6月于佳木斯大学附属第一医院神经内科就诊的缺血性脑卒中患者120例作为研究对象。使用CDR认知障碍功能检测后进行分组,分成缺血性卒中后非认知障碍组(A组)48例、缺血性卒中后轻度认知障碍组(B组)42例、缺血性卒中后痴呆(C组)30例。同时选择正常人群(40名)作为对照。分别检测不同组别人群血清中PGRN及SFRP5表达水平,同时使用MMSE量表对认知功能进行检测。对结果进行数据分析处理。结果 (1)四组患者血清中PGRN表达水平之间的差异有统计学意义(F=36.6,P=0.000)。组间两两比较,差异有统计学意义(P<0.05)。其中C组表达水平最高(38.39±6.75)pg/mL,D组表达水平最低(24.17±5.06)pg/mL。4组患者血清中SFRP5表达水平之间的差异有统计学意义(F=87.46,P=0.000)。组间两两比较,差异有统计学意义(P<0.05)。其中D组表达水平最高(8.03±1.14)μg/L,C组表达水平最低(4.67±0.69)μg/L。(3)急性缺血性脑卒中后的3组患者MMSE评分之间的差异有统计学意义(F=123.92,P=0.000)。其中以A组最高(26.31±3.57)分,C组最低(11.08±4.36)分。缺血性脑卒中后认知能力量表MMSE评分与PGRN呈负相关(r=-0.245,P=0.006),MMSE评分与SFPR5呈正相关(r=0.229,P=0.011)。结论 PGRN及SFRP5在缺血性卒中后认知障碍程度不同的患者血清中表达水平不同,且PGRN及SFRP5两者有助于认知障碍的早期评估与诊断。 Objective To investigate the level of PGRN and SFRP5 expression in the serum of patients with cognitive impairment after ischemic stroke and the correlation with cognitive impairment. Methods 120 cases of ischemic stroke in the Department of Neurology, First Affiliated Hospital of Jiamusi University, from July 2016 to June 2017 were selected as the research subjects. After using CDR cognitive dysfunction function test, the rats were divided into three groups: non cognitive impairment group(group A, 48 cases), mild cognitive impairment group(group B, 42 cases), dementia group(group C, 30 cases). The normal population(40 cases) was selected as the control. The levels of PGRN and SFRP5 in the serum of different groups were detected, and the cognitive function was detected by the MMSE scale.And the results were for data analysis. Results There was a significant difference between the levels of PGRN expression in the four groups of patients(F=36.6, P=0.000). The difference between the two groups was significant(P〈 0.05).The expression level of group C was the highest(38.39±6.75)pg/mL, and the expression level of group D was the lowest(24.17±5.06)pg/mL. There was a significant difference between the levels of SFRP5 expression in the four groups of patients(F=87.46, P=0.000). The difference between the two groups was significant(P〈 0.05). The expression level of D group was the highest(8.03±1.14)μg/L, and the expression level of group C was the lowest(4.67±0.69)μg/L.The difference in the MMSE score between the three groups after the acute ischemic stroke was obvious(F=123.92, P=0.000), and the MMSE score in the group A was the highest(26.31±3.57)points and in the group C was the lowest(11.08 ±4.36)points. The MMSE score was negatively correlated with the PGRN after the ischemic stroke(r=-0.245, P=0.006), and the MMSE score was positively correlated with the SFPR5(r=0.229, P=0.011). Conclusion PGRN and SFRP5 levels are different in serum of patients with different cognitive impairment after ischemic stroke, and PGRN and SFRP5 contribute to early assessment and diagnosis of cognitive impairment.
