开腹和腹腔镜保守性手术治疗异位妊娠患者围术期情况比较

Comparison of perioperative period of laparotomy and laparoscopic conservative surgery in patients with ectopic pregnancy

目的探究对异位妊娠患者实施开腹与腹腔镜保守性手术治疗的情况。方法择取我院2013年1月至2016年12月收治的120例异位妊娠患者作为研究对象,按数字奇偶法分为对照组和研究组,各60例。对照组给予开腹手术治疗,研究组应用腹腔镜保守性手术治疗,观察两组患者手术情况、并发症发生情况、输卵管再通率以及子宫再孕率。结果研究组患者的手术时间短于对照组,且术中出血量少于对照组(P<0.05)。研究组患者的术后肛门首次排气时间、术后首次下床活动时间及住院时间均明显短于对照组(P<0.05)。研究组患者的并发症总发生率低于对照组(P<0.05)。研究组患者的输卵管再通率及子宫再孕率高于对照组(P<0.05)。结论相比于开腹手术治疗,对异位妊娠患者应用腹腔镜保守性治疗,并发症较少,再次妊娠率高,治疗效果显著,值得临床推广。

Objective To explore the treatment of laparotomy and laparoscopic conservative surgery in patients with ectopic pregnancy. Methods A total of 120 cases of ectopic pregnancy patients admitted in our hospital from January 2013 to December 2016 were selected as the objects and divided into control group and study group according to the digital parity method, with 60 cases in each group. The control group was treated with laparotomy, the study group was treated with laparoscopic conservative surgery. The surgical conditions, complications, recanalization rates of the fallopian tubes and uterine re-pregnancy rate were observed between the two groups. Results The operative time of the study group was shorter than that of the control group, the intraoperative blood loss was less than that of the control group（ P〈0.05）. The first postoperative anus exhaust time, first postoperative activity out of bed time and hospital stay in the study group were significantly shorter than those in the control group（P〈0.05）. The total incidence of complications in the study group was lower than that in the control group（P〈0.05）. The fallopian tube recanalization rate and uterine re-pregnancy rate in the study group were higher than those in the control group（P 〈0.05）. Conclusion Compared with laparotomy, laparoscopic conservative surgery applied in patients with ectopic pregnancy has less complications, uterine re-pregnancy rate is higher,the treatment effect is remarkable, it is worthy of clinical promotion.
