单侧微小唇裂的临床处理原则探讨

Classification design and application of unilateral microform cleft lips

目的探讨单侧微小型唇裂分类设计手术的应用效果。方法按照Mulliken的分类法对57例微小型唇裂的患者进行分类,然后按照分类分别采用不同的手术方式进行修复。小型唇裂以改良Millard术式为主,部分病例应用了嵴上瓣;微型唇裂采用双"Z"成型术式;极微型唇裂以Rose切口修复。结果 54例患者创口一期愈合,有1例患儿术后出现皮内缝线排异反应,有2例鼻底出现轻度糜烂,术后随防3~12个月,唇弓形态恢复正常,唇红凹陷消除,唇部切口愈合良好,术后效果较满意。结论单侧微小型唇裂的分类设计能有效指导医生手术方式的选择。

Objective To explore the effect of classification design and operation of unilateral microform cleft lips repair.Methods Fifty-seven patients were classfied according to the classification of Mulliken and Repaired with different surgical methods. The minor cleft lip is mainly repaired by the modified Millard method,In some cases were applied with the Flap above Roll;Miniature cleft lip adopts the Doubleunilimb Z-plastic method;Extremely miniature cleft lip adopts the Rose method. Results 1 cases with suture rejection,2 cases of the nasal floor mild erosion,the others all I healing. Follow-up period 3-12 months,the effect satisfaction. Conclusion The classification design can effectively guide doctors to choice the correct approach to repair unilateral microform cleft lips.
