清胃散火汤辅助Vitapex糊剂根管充填治疗牙周-牙髓联合病变疗效及对PAI、PPD、CAL和炎性因子的影响

Effect of Qingwei Sanhuo Decoction and root canal filling with Vitapex paste on periodontal-endodontic combined lesions and its effects on PAI,PPD,CAL and inflammatory factors

目的观察清胃散火汤辅助Vitapex糊剂根管充填治疗牙周-牙髓联合病变疗效及对根尖周指数(PAI)、牙周探诊深度(PPD)、临床附着水平(CAL)和炎性因子的影响。方法将104例牙周-牙髓联合病变患者随机分为2组,对照组52例给予Vitapex糊剂根管充填治疗,研究组52例给予清胃散火汤辅助Vitapex糊剂根管充填治疗,2组均持续治疗8周。比较2组治疗后临床疗效,记录2组治疗前后相关牙周指数PAI、PPD和CAL变化情况,检测2组治疗前后血清炎性因子水平。结果治疗后,研究组临床治疗总有效率显著高于对照组(P<0.05);2组相关牙周指数PAI、PPD和CAL水平均较治疗前显著降低(P均<0.05),且研究组均显著低于对照组(P均<0.05);2组血清炎性因子肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-1β(IL-1β)水平均较治疗前显著降低(P均<0.05),且研究组均显著低于对照组(P均<0.05)。结论清胃散火汤辅助Vitapex糊剂根管充填治疗牙周-牙髓联合病变疗效显著,同时能明显改善相关牙周指数,降低炎性因子水平。

Objective It is to observe the effect of Qingwei Sanhuo Decoction and root canal filling with Vitapex paste in the treatment of periodontal-endodontic combined lesion and its influence on periapical index（ PAI）,periodontal detection depth（ PPD）,clinical attachment level（ CAL） and inflammatory factors. Methods One hundred and four patients with periodontal-endodontic combined lesion were randomly divided into 2 groups. Fifty-two patients in the control group were treated with Vitapex paste for root canal filling; and 52 cases in the study group were given Qingwei Sanhuo Decoction to assist Vitapex paste for root canal filling therapy. The treatment were continued for 8 weeks in the 2 groups. The clinical curative effect of the 2 groups after treatment was compared. The changes of periodontal index PAI,PPD and CAL were recorded,and the serum levels of inflammatory factors were detected in the 2 groups before and after treatment. Results After treatment,the total effective rate of the study group was significantly higher than that of the control group（ P〈 0. 05）. The PAI,PPD and CAL levels of the 2 groups were significantly lower than those before treatment（ all P〈 0. 05）,and the study group was significantly lower than that of the control group（ P〈 0. 05）. The levels of tumor necrosis factor-α（ TNF-α）,interleukin-6（ IL-6） and interleukin-1β（ IL-1β） in the 2 groups were significantly lower than those before the treatment（ P〈 0. 05）,and the study group was significantly lower than the control group（ P〈 0. 05）. Conclusion Qingwei Sanhuo Decoction and root canal filling with Vitapex paste are effective in the treatment of periodontal-endodontic combined lesion,and it can significantly improve the relative periodontal index,reduce the level of inflammatory factors.
