胸腺五肽联合清肺化浊汤治疗恶性肿瘤合并肺部感染疗效及对PCT、CRP、T淋巴细胞亚群水平的影响

Effect of thymopentin combined with Qingfei Huazhuo Decoction on malignant tumor with pulmonary infection and its influence on PCT,CRP and T lymphocyte subsets

目的观察胸腺五肽联合清肺化浊汤治疗恶性肿瘤合并肺部感染疗效及对降钙素原(PCT)、C反应蛋白(CRP)、T淋巴细胞亚群水平的影响。方法将100例恶性肿瘤合并肺部感染患者随机分为2组,对照组50例给予胸腺五肽治疗,观察组50例在此基础上加用清肺化浊汤治疗,记录2组临床症状体征改善时间,观察2组治疗前后主要症状评分和PCT、CRP及T淋巴细胞亚群水平,统计2组近期疗效。结果观察组体温恢复、血常规恢复、肺部啰音减轻、咳嗽咳痰减轻及肺部阴影吸收时间均显著低于对照组(P均<0.05);2组治疗后发热、咳嗽咳痰、气喘及啰音积分,PCT、CRP、CD3^+CD8^+及CD4^+CD25^+水平均显著降低(P均<0.05),CD3^+和CD3^+CD4^+水平均显著提高(P均<0.05),且观察组治疗后以上指标改善情况均显著优于对照组(P均<0.05);观察组治疗总有效率显著高于对照组(P<0.05)。结论胸腺五肽联合清肺化浊汤治疗恶性肿瘤合并肺部感染可有效加快病情康复进程,控制病情进展,降低炎性反应水平,并有助于提高机体免疫系统功能。

Objective It is to observe the curative effect of thymopentin combined with Qingfei Huazhuo Decoction in the treatment of malignant tumor with pulmonary infection and its influence on the level of procalcitonin（ PCT）,C-reactive protein（ CRP） and T lymphocyte subsets. Methods One hundred cases of patients with malignant tumor complicated with pulmonary infection were randomly divided into 2 groups. 50 cases in the control group were treated with thymopentin,50 cases in the observation group were treated with Qingfei Huazhuo Decoction combined with thymopentin. The improvement time of clinical symptoms and signs were recorded; the main symptom score,the level of PCT,CRP and T lymphocyte subsets before and after treatment was observed in the two groups; the short-term effects of the two groups were analyzed. Results The time of temperature recovery,blood routine recovery,lessen of lung rale,relieving of cough and expectoration,the absorption of lung shadow of the observation group were significantly lower than those of the control group（ all P〈 0. 05）. After treatment,the levels of PCT,CRP,CD3＋CD8＋and CD4＋CD25＋were significantly decreased but the levels of CD3＋and CD3＋CD4＋were significantly increased（ all P〈 0. 05） in the 2 groups,and the improvement of the above indexes in the observation group was significantly better than that in the control group（ P〈 0. 05）. The total effective rate of the observation group was significantly higher than that of the control group（ P〈 0. 05）. Conclusion Thymopentin combined with Qingfei Huazhuo Decoction in treating malignant tumor with pulmonary infection can effectively speed up the rehabilitation process,control the progress of the disease,reduce the level of inflammatory response,it also helps to improve the function of the body＇s immune system.
