摘要
介绍以门诊电子病历为入口的分级诊疗建设探索经验,结合医院和社区现有信息化基础,通过门诊电子病历系统整合医疗数据对分级诊疗就医模式形成支撑,通过创新设计对分级诊疗业务进行实践探索和特色应用,为患者便捷就医和分级诊疗的实践落地提供参考。
This paper introduced the practice of graded diagnosis and treatment construction via outpatient electronic medical records combined with the existing hospital and community information foundation. Integrating medical data of graded clinics through the outpatient electronic medical records system could support the graded diagnosis and treatment. The creative design provides reference for the landing of graded disgnosis and treatment and facilitates patients in the treatment.
