摘要
目的:探讨新活素对老年糖尿病合并急性心肌梗死患者心功能的影响。方法:选取2015年8月~2017年8月间某院收治的120例老年糖尿病合并急性心肌梗死患者,患者入院前或入院时伴有急性心力衰竭,随机分为观察组和对照组,观察组在常规治疗基础上给予新活素,对照组在常规治疗基础上给予硝酸甘油,比较两组治疗72h后疗效,血肾素、血管紧张素II、醛固酮及治疗前后左室舒张末期容积(LVEDV)、左心室射血分数(LVEF)以及治疗过程中的不良反应。结果:观察组治疗效果优于对照组(95.67%VS 74.38%,P<0.05),观察组肾素、血管紧张素II、醛固酮水平均低于对照组(P<0.05),治疗前后1周超声心动图显示两组LVEDV的改变无统计学意义(P>0.05),观察组LVEF相较于对照组明显改善(P<0.05)。结论:新活素可以明显改善老年糖尿合并急性心肌梗死患者的心功能,且疗效显著,不良反应少。
Objective: To investigate the effect of rh-BNP on cardiac function in elderly patients with diabetes mellitus complicated with acute myocardial infarction. Methods: From August 2015 to August 2017, 120 elderly patients with diabetes mellitus complicated with acute myocardial infarction treated in a hospital were randomly divided into observation group and control group.The observation group was given rh-BNP with basic routine treatment, and the control group was given nitroglycerin with basic routine treatment. After 72 hours of treatment, The therapeutic effects, serum renin, angiotensin II, aldosterone, left ventricular end-diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), and the adverse reactions in the course of treatment were compared between the two groups. Results: The therapeutic effect of the observation group was better than that of the control group (95.67% VS 74.38%, P 〈0.05), the serum levels of renin, angiotensin II and aldosterone in the observation group were lower than those in the control group ( P 〈0.05), there was no significant difference in LVEDV between the two groups by echocardiography before and after treatment ( P 〉0.05),and the LVEF of the observation group was obviously improved compared to control group ( P 〈0.05). Conclusion: The rh-BNP can significantly improve heart function of the elderly diabetic patients with acute myocardial infarction, and has little adverse reaction.
