抗生素骨水泥对人工膝关节置换术后患者炎性因子的影响研究

摘要 目的探讨抗生素骨水泥对骨性关节炎患者行人工膝关节置换术后炎性因子的影响。方法选取2013年2月至2016年6月在我院行人工膝关节置换术的骨性关节炎患者80例,随机分为两组各40例。试验组患者术中使用骨水泥添加1.0 g注射用盐酸万古霉素,对照组患者使用不添加抗生素的骨水泥,观察两组的疗效及术后炎性因子变化情况。结果术后3天、7天,两组的ESR、CRP水平较治疗前下降,试验组的ESR、CRP水平显著低于对照组(P<0.05)。试验组的术后卧床时间、感染发生率均显著低于对照组,屈膝角度显著大于对照组(P<0.05)。试验组的总有效率为97.5%,显著高于对照组的80.0%(P<0.05)。结论使用抗生素骨水泥可有效地预防和减少骨性关节炎患者行人工膝关节置换术时的感染,提高治疗效果。 Objective To investigate the effect of antibiotic bone cement on inflammatory factors after artificial knee arthroplasty in patients with osteoarthritis. Methods 80 cases of osteoarthritis patients with artificial knee arthroplasty in our hospital from February 2013 to June 2016 were selected and randomly divided into two groups, with 40 cases in each group. The experimental group was treated with bone cement with 1.0 g vancomycin hydrochloride for injection during the operation, while the control group was treated with bone cement without antibiotics. The therapeutic effects and changes of inflammatory factors after surgery of two groups were observed. Results 3 days and 7 days after surgery, both groups had significantly decreased levels of ESR and CRP, and the levels of ESR and CRP of the experimental group were significantly lower than those of the control group(P〈0.05). The postoperative bedridden time and incidence of infection of the experimental group were significantly lower than those of the control group, and the range of flexion was significantly higher than that of the control group(P〈0.05). The total effective rate of the experimental group was 97.5%, significantly higher than 80.0% of the control group(P〈0.05). Conclusions The use of antibiotic bone cement can effectively prevent and reduce the infection in patients with osteoarthritis during artificial knee arthroplasty, and improve the therapeutic effect.
