术前CT测量椎体变异对脊柱侧弯后路矫形手术的意义

Significance of preoperative CT measurement of vertebrae variation in posterior orthopedic surgery of scoliosis

目的探讨术前CT测量椎体变异对脊柱侧弯后路矫形手术效果的影响。方法选择我院2015年1月至2016年12月在我院接受后路矫形手术治疗的70例脊柱侧弯患者为研究对象。回顾患者资料,将术前接受CT测量椎体变异的35例归入观察组,未接受CT测量的35例归入对照组。比较两组患者的手术效果。结果观察组和对照组椎弓根钉使用率分别为(86%±14%)、(64%±18%),组间差异具有统计学意义(P<0.05)。观察组和对照组患者治疗总有效率分别为97.14%、94.29%,组间差异无统计学意义(P>0.05)。术后1年,两组患者脊椎Cobb's角均显著改善,且观察组优于对照组(P<0.05)。结论对于脊柱侧弯患者,可于术前先行CT测量椎体变异的情况,以指导术中固定植入,提高手术效果,值得推广。

Objective To investigate the effect of preoperative CT measurement of vertebrae variation in posteriororthopedic surgery of scoliosis. Methods Seventy patients with scoliosis treated by posterior orthopedic surgery from January2015 to December 2016 in our hospital were selected. The patients＇ data were reviewed. Thirty-five patients vertebralvariation measured by CT before operation were included in the observation group, and those who were not were includedin the control group. The surgical effects of the two groups were compared. Results The utilization rate of pedicle nail inthe observation group and control group was （86%±14%） and （64%±18%） respectively, the difference between the twogroups was statistically significant （P〈0.05）. The effective rate of treatment was 97.14% and 94.29% in the observationgroup and control group respectively, there was no significant difference between the two groups （P〉0.05）. At 1 year afteroperation, the spinal Cobb＇s angles of both groups significantly improved, and that of the observation group was superior tothe control group （P 〈0.05）. Conclusion For patients with scoliosis, the changes of vertebrae can be measured bypreoperative CT. It can guide implanting in operation and improve the effect of operation, which is worthy of promotion.
